Ziele und Ergebnisse extraoraler Züge in postero-anteriorer Richtung in Anwendung einer orthopädischen Maske bei der Behandlung von Fällen der Klasse III

Zusammenfassung

Die Vorschwenkung des Oberkiefers, die Mesialverschiebung des oberen Alveolar- und Zahnbogens und die Anpassung der Mandibula an den neuen Zustand des Oberkieferkomplexes können durch extraorale Züge in postero-anteriorer Richtung unter Stirn-Kinnabstützung erzielt werden. Diese Veränderungen sind verschieden nach den vorliegenden Verhältnissen. Sie hängen vom Alter des Patienten, Art der Anomalie und Gebißzustand ab und auch von der durchgeführten Behandlung, wie Richtung, Stärke und Anwendungsdauer der Züge. Bei den Patienten mit Fehlbildungen im Zusammenhang mit Lippen-Kiefer-Gaumenspalten läßt sich durch extraorale Kräfte in divergenter postero-anteriorer Richtung auch leicht die bestehende Oberkieferkompression behandeln. Diese Kräfte greifen sehr hoch an und wirken bis zur Verbindung der Oberkiefer mit dem Os frontale. Wesentliche Kennzeichen der extraoralen Züge in postero-anteriorer Richtung sind also die gute Wirksamkeit und die Vielheit ihrer Auswirkungen. Es ist jedoch wesentlich festzustellen, daß die so erzielten Ergebnisse erst dann beständig sind, wenn die verlagerten Organteile wieder ein funktionelles Gleichgewicht gefunden haben. Eine Reedukation der oro-fazialen Funktionen und selbst (in Fällen von begleitenden Fehlbildungen bei Lippen-Kieferspalten) chirurgische Eingriffe vor allem der Weichteile können daher notwendig sein. Die Beständigkeit der erzielten Ergebnisse und die gute Wirksamkeit der Behandlung sind jedenfalls zwei verschiedene, wenn auch zusammenhängende Probleme.

Summary

Heavy postero-anterior extra-oral forces exerted by means of a front-chin support can cause:

• 1- A forward rocking motion of the maxilla,

• 2- a mesial shift of the maxillary dentoalveolar arch,

• 3- an adjustment of the mandible in consideration of the modified state of the maxillary structures.

The modifications achieved by the posteroanterior extra-oral forces depend on several factors:

• 1- Factors which are inherent in the patient himself (age, type of dysmorphosis, state of dentition on which the tractions are performed),

• 2- Factors according with the chosen treatment (direction, intensity and duration of the applied forces).

In hare-lip and cleft-palate cases, posteroanterior laterally oriented extra-oral forces allow an easy treatment of the endognathia. These forces work at a high level, even at the level of the very end of the frontal process of the maxilla, where this end is implanted on the frontal bone.

There fore some of the most salient features of the postero-anterior extra-oral forces are the efficiency and the multipurposal use.

However it is important to conceive that the results one could obtain are stable only if the conditions of the equilibrium of the displaced structures are modified. Particularly oro-facial functions have to be re-educated if necessary and, even (in hare-lip and cleft-palate sequellae cases) some operations could be performed, eventually, specially on soft tissues.

Résumé

La bascule antérieure du maxillaire, la mésiogression de l'arcade alvéolo-dentaire supérieure, et l'adapistion de la mandibule au nouvel état des structures maxillaires peuvent être obtenues par les tractions extraorales postéroantérieures sur appui fronto-mentonnier. Elles varient selon diverses conditions dépendant du sujet traité (âge, variété de la dysmorphose, état de la denture qui supporte l'ancrage) et aussi selonle traitement effectué (sens des tractions, force et durée de celles-ci). Chezles sujets atteints de séquelles des fentes labio-maxillo-palatines, les forces extra-orales postéro-antérieures divergentes permettent de réduire aisément l'endognathie maxillaire. Ces forces agissent très haut, jusqu'à l'implantation frontale des os maxillaires supérieurs. Une des principales caractérististiques des tractions extraorales postéro-antérieures est done l'efficacité et la multiplicité de leurs actions. II est essentiel, néanmoins, de bien savair que les résultats ainsi obtenus ne sont stables que si les conditions d'équilibre des structures déplacées zont modifiées. Une rééductation des fonctions oro-faciales et, même, (en cas de séquelles de fentes labio-maxillaires) certaines interventions chirurgicales portant notamment sur les tissus mous peuvent donc étre indispensables. Stabilité des résultats obtenus et efficacité du traitement sont toutefois deux problèmes différents (bien que connexes).
