Zusammenfassung
Diese Untersuchung basiert auf den Anfangs- und Schlußfernröntgenbildern von 30 sowohl dentalen als auch skelettalen Klasse-II. 1-Fällen. Von diesen Fällen wurden 10 mit dem Aktivator und 10 mit dem Zervikalheadgear behandelt; 10 unbehandelte Fälle dienten als Kontrollgruppe. Zuerst wurden die Auswirkungen beider Apparaturen auf das Gesichtsskelett mit dem Vergleich von Aktivatorgruppe und Kontrollgruppe sowie Zervikalheadgear-Gruppe und Kontrollgruppe getrennt untersucht. Dann wurden die beiden Behandlungsmethoden verglichen. Es wurden folgende signifikante Unterschiede gefunden: Der Aktivator retrudiert die oberen Frontzähne, wo der Zervikalheadgear keinen signifikanten Einflu\ hat. Der Zervikalheadgear distalisiert und extrudiert die oberen Molaren und kippt die OK-Basisebene, wobei die Wirkung des Aktivators nicht signifikant ist. Die mit dem ANB-Winkel gemessene sagittale Diskrepanz zwischen dem Oberkiefer und dem Unterkiefer wurde mit beiden Behandlungsmethoden verkleinert. Die Verkleinerung des ANB-Winkels ist bei Anwendung des Zervikalheadgears im Gegensatz zum Aktivator verdoppelt, da der Zervikalheadgear auf den OK-Komplex dorsal einen signifikanten Einfluß hat. In der Wirkung der beiden Behandlungsmethoden auf das sagittale und vertikale Wachstum des Unterkiefers wurde kein Unterschied festgestellt. Der Unterkiefer setzte sein normales Wachstum in zwei Richtungen fort.
Summary
This study is based on pre- and post-treatment cephalographs of 30 cases with class II division 1 malocclusions on skeletal II dental bases. 10 cases were treated with an activator, 10 with cervical headgear, and 10 non-treated cases served as a control group. First, in order to determine the actual effects of the activator and cervical headgear on the facial skeleton, each treatment group was compared to the control group separately. Subsequently, the group treated with an activator was compared to the group treated with cervical headgear. The significant differences between the two treatment methods were as follows: The activator retracted the upper incisors whereas the cervical headgear showed no significant effect. However, the cervical headgear distalised the upper molars, extruded them, and tilted the maxillary base, whereas the effect of the activator was insignificant. The sagittal skeletal discrepancy, measured by the ANB angle, was reduced by both methods. Reduction of the ANB angle using cervical headgear however, was twice that of the activator due to the influence of the cervical headgear on the maxillary complex. No difference was found between the two methods with respect to the vertical and sagittal growth pattern of the mandible. Thus, the mandible continued its normal growth patttern in a downward and forward direction.
Resumé
Cette étude, basée sur l'examen de téléradiographies prises au début et à la fin du traitement, compren l'examen de 30 cas présentant une classe II/1 aussi bien squelettique que dentaire. On a soigné 10 patients par activateur, 10 autres avec un headgear cervical, les 10 derniers cas formant le groupe témoin. Pour étudier l'action de l'appareillage sur le squelette facial, on a d'abord comparé chaque groupe traité avec le groupe témoin, puis on a comparé entre elles, les deux méthodes de traitement. On a alors trouvé les différences suivantes: L'activateur a produit une rétrusion des incisives supérieures mais pas le headgear cervical. Par contre, celui-ci a provoqué la distalisiation et l'extrusion des molaires supérieures ainsi qu'une rotation de la base du maxillaire tandis que l'action de l'activateur n'est pas significative à ce sujet. Le décalage sagittal entre les deux maxillaires, mesuré par l'angle ANB est diminué par les deux méthodes de traitement. La diminution obtenue par le headgear est deux fois plus élevée que celle produite par l'activateur; cela indique que le headgear cervical est plus efficace sur le maxillaire dans le plan sagittal. Par contre, on n'a pas décelé de différences entre les deux modes thérapeutiques sur la croissance sagittale et verticale de la mandibule; celle-ci a continué normalement sa croissance dans ces deux plans.
