Zusammenfassung
1.Durch die Verwendung eines Hochzuges bei der Headgear-Aktivator-Therapie schwanken die Kräfte maximal um 20% während der Nacht. 2. Nach einer unterschiedlich langen Gewöhnungszeit an die Behandlungsgeräte wurden keine Veränderungen an der Schlafdauer festgestellt. 3. Während der Nacht wird nur selten so auf den Aktivator zugebissen, daß die therapeutische Stellung erreicht wird. 4. Nach längerer Tragezeit kommen häufigere Zubisse in der therapeutischen Lage vor. 5. Die weitaus häufigsten Zubisse jedoch finden bei gut adaptierten Patienten in der Nähe der therapeutischen Stellung statt, während der Gewöhnungsphase sind die Zubisse weit von der therapeutischen Lage entfernt. 6. Durch die Verklemmungssituation des Aktivators sind die Zubisse sowohl in vertikaler als auch in transversaler Richtung wirksam. Eine Fixierung des Aktivators am Oberkiefer und ein exaktes Einschleifen erleichtern das Einnehmen der therapeutischen Okklusion. 7. Eine Überkorrektur beim Konstruktionsbiß ist sinnvoll, da nur wenig Zubisse in der gewünschten Position erfolgen. 8. Ein zu großzügiges Beschleifen des Aktivators im Seitenzahnbereich führt zu einer Protrusion der Unterkieferschneidezähne und zur Retrusion der Oberkieferfront. 9. Zu große sagittale Vorverlagerung des Unterkiefers erschwert die Gewöhnung an das Gerät. 10. Dauerkräfte, durch Muskelaktivitäten hervorgerufen, treten während der Nacht nicht auf. 11. Die nächtlichen Muskelaktivitäten sind jeweils mit Pausen nur wenige Sekunden lang wirksam, dann nimmt der Unterkiefer eine neue, größere Abstandshaltung ein.

1.While using high-pull headgear in the headgear activator therapy, fluctuations in the extraoral forces during the night are less than 20%. 2. After the appliance has been accepted and tolerated by the patients, sleeping-time is not affected. 3. During the night patients seldom bite on the activator, in the therapeutic occlusion. 4. After wearing the appliance for a long time biting into the therapeutic occlusion occurs more frequently. 5. In most cases we find that well-adapted patients bite near the therapeutic occlusion. At the onset of treatment, however, biting actions are short of the therapeutic occlusion. 6. On account of the activator's binding action of the teeth, biting produces forces in both the vertical and transversal directions. Fixing the appliance on the upper jaw and careful grinding will bring the lower jaw into a position nearer that of the therapeutic occlusion. 7. Over-correction of the working bite is necessary since only a few biting actions take place in the desired position. 8. Exaggerated filing out of the molar and premolar region will tend to procline the lower incisors and retrocline the upper incisors. 9. Construction bites taken in an extreme sagittal position will hinder the patients' tolerance of the appliance. 10. Continuous force from muscle activity was not established during the night. 11. Normally the muscle activity acts for short intervals — some seconds. Thereafter, the lower jaw assumes a new increased position relative to the appliance.
Résumé
1.Avec l'usage du «high-pull» au cours de la thérapie activateur-headgear, les forces varient au maximum de 20% pendant la nuit. 2. Après une phase d'adaptation plus ou moins longue, on n'a pas enregistré de modifications dans la stabilité du sommeil. 3. Pendant la nuit, on observe rarement que l'enfant propulse sa mandibule de manière à atteindre la position thérapeutique. 4. Avec le temps, l'enfant s'adapte et propulse davantage dans la position thérapeutique désirée. 5. Grâce à la construction bien adaptée de l'activateur, il s'opère aussi un effet thérapeutique en direction verticale et transversale. 6. II est recommandable de positionner la mandibule assez en avant afin que les enfants la propulsent comme il est désiré. Mais si on l'hyperpropulse trop, cela devient difficile pour l'enfant d'avancer autant sa mandibule. 7. Une bonne fixation de l'activateur au maxillaire et un meulage exact rendent la propulsion mandibulaire plus correcte. 8. En cas de meulage trop massif de la résine, on pourra observer une protrusion des incisives mandibulaires et une rétrusion des incisives maxillaires. 9. Des forces musculaires ne s'exercent pas de façon permanente au cours de la nuit. 10. Les activités musculaires nocturnes apparaissent seulement pendant quelques secondes après que la mandibule reprenne un nouveau positionnement.
