Zusammenfassung
Es wird eine Methode zur Bestimmung des Herzminuten- und Schlagvolumens beschrieben, die ein dichromatisches Densitometer zur fortlaufenden Registrierung von Indocyanin-grün (Cardiogreen) im strömenden Blut verwendet. Das Gerät zeigt den Farbstoffgehalt des Blutes unabhängig von Veränderungen der Blutströmung, des Hämatokrits, der Kohlensäurespannung und verschiedener anderer Faktoren an. Die Farbstoffverdünnungskurve wurde nach dem Vorgehen vonSparling et al. mit Hilfe der sog. dynamischen Eichung ausgewertet. Sauerstoffbeatmung und Reinfusion des Blutes entfielen. Gleichzeitig mit der Farbstoffkonzentration des Blutes wurde das Ekg geschrieben.
Doppel- bzw. Dreifachbestimmungen des Herzminuten- und Schlagvolumens ergaben eine absolute Abweichung von 8,5–11% der gemessenen Einzelwerte. Der Korrelationsfaktorr errechnete sich mit 0,93 für das Schlag- und Herzminutenvolumen.
Die Untersuchungen wurden an insgesamt 46 Personen durchgeführt. Neben Herz- und Kreislaufgesunden wurden Patienten mit angeborenen und erworbenen Herzfehlern und verschiedenen anderen kardiovaskulären Erkrankungen herangezogen. Auch werden Herzminuten- und Schlagvolumenbestimmungen bei selteneren Krankheitsbildern mitgeteilt.
Obgleich die Methode eine Vereinfachung bisheriger Verfahren der Küvettendensitometrie darstellt, zeichnet sie sich durch statistisch nachgeprüfte Zuverlässigkeit bei geringerer belastung der Untersuchten aus.
Summary
A technic for measurement of the cardiac output and stroke volume is described, using a dichromatic densitometer for the continuous determination of indocyanine green in the flowing blood. Previous studies reported that the densitometer records the dye concentration independent from changes in blood flow, hematocrit, CO2-tension and various other factors. Calibration of the dye dilution curves was obtained by the “dynamic” procedure ofSparling et al. Oxygen breathing and reinfusion of blood was not needed. Simultanously with the dye dilution curve the electrocardiogramm was registered.
Double and triple determinations of the cardiac output and of the stroke volume showed an absolute deviation of 8,5 to 11%. The correlation factorr=0.93.
The studies were performed on 46 persons. Besides control subjects having no cardiovascular disease there were patients with congenital and acquired heart lesions and various other cardiovascular abnormalities. Measurement of the cardiac output and stroke volume on other rare diseases are reported.
Although the method described means a simplification of hither known procedures on cuvette densitometry, it proves a good reliability on statistical evaluation and less annoyance to the probee.
