Abstract
Among the great number of environmental irritants which act on the respiratory tract, smoking and air pollution have acquired a special significance for triggering and deterioration of bronchial diseases. An experimentally well defined reaction to exogeneous stimuli is acute reflex bronchoconstriction. In a study on 36 normal persons, 31 patients with chronic bronchitis, and 24 patients with exogen-allergic asthma, the effect of inhalation of cold air (0° to −2° C for 10 min) was compared to the effect of tobacco smoke inhalation (filter-tipped cigarette for 4 min). The increase of airway resistance (Raw) measured by body plethysmography was largely dependent on individual factors and on pre-existing bronchial diseases only to a lesser degree on the type of irritant. The mean increase in airway resistance after tobacco smoke, as after cold air, was about 2-fold in the bronchitis group and 5-fold in the asthma group as against the reaction of normal controls (for details see table). Etiology and pathophysiology of the varying individual irritability of the bronchial tree are discussed.
Zusammenfassung
Auf eine Fülle von Umweltreizen reagiert der Tracheobronchialbaum mit einer Erhöhung seines Strömungswiderstandes. Dieser Effekt ist überwiegend Ausdruck einer Reflexbronchokonstriktion. Die Reaktionsstärke hängt weniger von der Art des Reizes als von individuellen Faktoren und von vorbestehenden Bronchialerkrankungen ab. Am Beispiel der Tabakrauch- und der Kaltluftbronchokonstriktion wird gezeigt, daß Asthmapatienten viel empfindlicher reagieren als Lungengesunde und häufig die Erhöhung des Bronchialwiderstandes bereits subjektiv als Atemnot empfinden. Die mutmaßlichen Ursachen einer erhöhten bronchomotorischen Reizbarkeit durch Umweltfaktoren werden diskutiert.
