Pulssynchroner Pendelnystagmus, Fistelsymptome ohne Fistel und Lagefistelsymptom

Zusammenfassung

  1. 1.

    Es wird über 2 Kranke berichtet, die bei intakten Trommelfellen und negativer WaR in Blut und Liquor Fistelsymptome ohne Fistel und—was u. W. bisher nicht beschrieben ist—pulssynchronen Pendelnystagmus aufwiesen.

  2. 2.

    Vasculäre und pressorische Fistelsymptome wirkten gegensinnig hinsichtlich der Schlagrichtung des Nystagmus.

  3. 3.

    Vasculäre und pressorische Fistelsymptome ließen sich in dem einen Falle durch gleichzeitige Prüfung aufheben, im anderen wurde dies nicht geprüft.

  4. 4.

    In dem 2. Fall führte Lagewechsel, der endocranielle Druckerhöhung erzeugte, zu einem rotatorischen Nystagmus nachrechts, also zum gesunden Ohr, und Lagewechsel, der endokranielle Druckentlastung bewirkte, zu einem rotatorischen Ruck-Nystagmus nachlinks, also zum kranken Ohr. Der Nystagmus trat jeweils anfallsweise für einige Sekunden nach Einnehmen der Lage auf. Für diesen Sonderfall eines Lagenystagmus wird die Bezeichnung “Lagefistelsymptom” vorgeschlagen. Im vorliegenden Falle handelt es sich um ein “Lagefistelsymptom ohne Fistel”.
