Summary
It was the purpose of the first study of the “Wissenschaftliche Arbeitsgemeinschaft für die Therapie von Lungenkrankheiten” (W.A.T.L.) to find a practicable method for the early clinical evaluation of new antituberculosis drugs. For this reason thiocarlide (DATC) was tested in a controlled clinical trial on a short-term monotherapeutic basis and compared with PAS and bedrest. Adults with previously treated cavernous pulmonary tuberculosis and high bacterial sputum counts were treated at random for 6 weeks with one of the three regimens. Two weeks before the beginning of treatment and during treatment bacterial counts in enriched sputum specimens were carried out two times a week, using the method ofGaffky in a modification which takes into consideration the true bacterial concentration, called Gaffky code. A total of 29 patients was evaluated. Apart from the overall groups special “homogenized” groups were set up. These latter groups were formed by elimination of all patients who could not be adequately matched with partners managed under the other treatment plans. Two matching criteria were used:

the largest diameter of the largest cavity and
-
2.
the so-called cavity-index.
The trial revealed significant results by using the Wilcoxon test in all three groups at a level of significance ofp=0,01, demonstrating that PAS is better than DATC (Isoxyl) and DATC is better than bedrest alone.
Other criteria, f.i. growth of cultures, reduction of cavity size, regression of sedimentation rate confirmed the results although they were not statistically significant.
Zusammenfassung
In einer kontrollierten Prüfung wurde DATC mit PAS in 6wöchiger Monotherapie verglichen. Eine dritte Patientengruppe wurde nur mit Bettruhe behandelt.
Zur Prüfung zugelassen wurden erwachsene Patienten mit vorbehandelter kavernöser Lungentuberkulose, die anfänglich bei wiederholter Untersuchung reichlich Tuberkulosebakterien ausschieden und deren Tuberkulose sich unmittelbar vor Beginn der Prüfung nicht regredient verhalten hatte.
Als Beurteilungskriterium galt das Verhalten der nach Anreicherung im Sputum festgestellten Bakterienkonzentration. Die Bakterienzählung erfolgte zweimal wöchentlich. Ein anderes Kriterium war das Kavernenverhalten.
Es erwies sich, daß die Prüfungsanordnung aussagekräftige Ergebnisse lieferte, welche einer statistischen Analyse standhalten. Auch der Wert zweitrangiger antituberkulöser Stoffe läßt sich mit der verwendeten Methode ermitteln. Speziell ergab sich, daß in den ersten 6 Wochen PAS wirksamer ist als Isoxyl und Isoxyl wirksamer als Bettruhe allein. Diese Ergebnisse der bakterioskopischen Untersuchung konnten mit einer Vertrauensgrenze vonP=0,01 statistisch gesichert werden, und zwar sowohl beim Vergleich der Gesamtgruppen als auch beim Vergleich sogenannter homogenisierter Gruppen. Mit dieser Homogenisierung wurde eine noch bessere Angleichung der Anfangsbefunde in den Gruppen angestrebt. Diese Angleichung erfolgte einmal nach dem sogenannten „Kavernenindex“ und zum zweiten nach dem Durchmesser der größten Kaverne und der Kavernenzahl. Andere Untersuchungsergebnisse, wie kulturelle Wachstumsstärke, Kavernenverkleinerung, Rückgang der BSG, bestätigten unsere Feststellungen, waren aber nicht statistisch signifikant.
Das erst nach längerer Anlaufzeit und schwächer als PAS wirksame Thiocarlid (DATC) kann nicht als gleichwertiger Ersatz für PAS in der Anfangsphase der Behandlung empfohlen werden.
