Zusammenfassung
Das Mesotheliom ist seit 1975 der häufigste Berufskrebs in der Schweiz (ca. 70 %). 1969 bis 1982 registrierte die SUVA 49 Fälle, davon 10 mit Asbestose. Analysen der ersten 30 Beobachtungen bis 1980: 29 pleural, 1 peritoneal Kausaler Asbestkontakt bei: Asbestzementproduktion 9, Waggonbau und -reparaturen 6, Chemie (Isolationen) 5, diverse 10. Expositionsdauer Ø 21 Jahre. Latenzzeit zwischen Exposition und Diagnose Ø 35 Jahre. Ueberlebenszeit nach Diagnose Ø 1 Jahr. Technische Prophylaxe: Substitution, Feinstaubvermeidung bzw. -verminderung unter MAK. Medizinische Prophylaxe: Eignungs- und Nachuntersuchungen. Hinweis auf Meldung an Mesotheliom-Register in Zürich und an SUVA zwecks Abklärung beruflicher Ursache.
Résumé
Le mésothéliome est depuis 1975 le plus fréquent des cancers professionnels en Suisse (environ 70 %). De 1969 à 1982 la Caisse nationale a enregistré 49 cas dont 10 avec asbestose. L'analyse des premiers 30 cas jusqu'à 1980 est la suivante: 29 mésothéliomes pleuraux, 1 péritonéal. Contact professionnel avec l'asbeste: 9 dans la fabrication du fibrociment, 6 dans la fabrication et la réparation de wagons de chemins de fer, 5 dans l'industrie chimique (isolation), 10 dans diverses conditions. La durée moyenne d'exposition est de 21 ans, le temps de latence entre l'exposition et le diagnostic de 35 ans, la survie après le diagnostic 1 an. La prévention technique consiste en la substitution, la diminution des concentrations en poussière fine au-dessous de la valeur MAC. La prévention médicale consiste en examens d'entrée et de contrÔle. Enfin l'auteur suggère qu'on signale les cas de mésothéliome au registre du mésothéliome à Zurich et à la Caisse nationale qui se chargera de l'établissement de l'anamnèse professionnelle.
Summary
From 1975 onwards, mesothelioma is by far the most often seen occupational tumour in Switzerland (about 70 %). Between 1969 and 1982, there have been 49 cases registered by the SUVA (SchweizerischeUnfallversicherungsanstalt, i.e. Swiss National Accident Insurance Fund). 10 together with asbestosis. Of the first 30 cases, the primary tumour only once was in the peritoneum, in all other cases the tumour always could be located in the pleura. The causative asbestos exposure was found in the production of asbestos cement (9), in the construction and reparation of waggons (6), in the insulation work at chemical plants (5) and in several other industrial work conditions (10). The average exposure time was 21 years. The average interval from exposure to diagnosis 35 years. The survival time after diagnosis about 1 year. Technical prevention consists in substitution of asbestos or elimination of respirable dust (or at least reduction below the TLV). Medical prevention: periodic fitness tests and follow up examinations. New cases should be communicated to the mesothelioma-register in Zurich and to the SUVA, for clarification of the industrial origin.
