Abstract
A total of 170 patients undergoing upper gastrointestinal surgery requiring a jejuno-jejunal enteroanastomosis were randomized into two groups according to the method to be employed at the enteroanastomosis. The most common procedures were partial gastrectomy for benign disease (84 patients), partial or total gastrectomy for malignant disease (35 patients), and bilioenteral bypass (31 patients, 25 for malignant disease). The enteroanastomosis was created by manual suture (3-0 Dexon®, two layers) in 89 patients and with the Biofragmentable Anastomosis Ring (Valtrac-BAR®) in 81 patients. Both end-to-side (101 patients) and side-to-side (69 patients) reconstructions were done. No ruptures or obstructions of the jejuno-jejunal anastomosis occurred. The postoperative recovery of the gastrointestinal tract was similar in the two groups in duration of nasogastric drainage, intravenous fluid therapy, and intestinal paralysis. Nor was there any difference between the groups in the duration of the postoperative hospital stay. Ten patients, 7 in suture-group and 3 in BAR-group, died postoperatively of causes unrelated to the enteroanastomosis. Thus the mortality rate was 7.9% in the suture group and 3.7% in the BAR group. The biofragmentable ring offers a safe alternative to manual sutures for small bowel anastomoses.
Résumé
Par une étude randomisée, on a comparé les anastomoses du tube digestif supérieur par des anneaux biofragmentables Valtrac-BAR à des sutures manuelles chez 170 patients. Il y a eu 84 gastrectomies partielles pour maladies bénignes, 35 gastrectomies partielles ou totales pour maladies malignes et 31 anastomoses biliodigestives dont 25 pour des maladies malignes. L'anastomose manuelle a été faire en deux couches au Dexon 3-0 chez 89 patients et avec les anneaux biofragmentables (Valtrac-BAR) chez 81 patients. On a inclu les anastomoses termino-latérales (n=101) et latéro-latérales (n=69). Il n'y a eu aucune fistule ou sténose parmi les anastomoses jéjunojénunales. Les suites postopératoires ont été comparables dans les deux groupes en ce qui concerne la durée de l'aspiration nasogastrique, des perfusions intraveineuses et de l'iléus intestinal, ainsi que la durée d'hospitalisation postopératoire. Il y a eu 10 morts, 7 dans le groupe manuel, 3 dans le groupe Valtrac-BAR, toutes sans rapport avec l'anastomose. Le taux de mortalité était de 7.9% dans le groupe manuel et 3.7% dans le groupe Valtrac-BAR. Les anneaux biofragmentables sont une alternative sûre aux anastomoses manuelles pour les anastomoses de l'intestin grêle.
Resumen
Un total de 170 pacientes sometidos a cirugía sobre el tracto gastrointestinal alto que requiriera anastomosis yeyuno-yeyunal fue asignado en forma aleatoria a un grupo en el cual se uitilizó el anillo anastomótico Valtrac-BAR o a un grupo en que se utilizó sutura. El procedimiento más frecuente fue la gastrectomía realizada por enfermedad benigna (84 pacientes), gastrectomía parcial o total por neoplasia maligna (35 pacientes) y derivación biliontérica (31 pacientes, 25 por neoplasia maligna). La enteroanastomosis fue construida mediante el anillo anastomótico biofragmentable (Valtrac-BAR) en 81 pacientes. Las anastomosis fueron de tipo término-lateral (101 pacientes) y látero-lateral (69 pacientes). No se presentaron rupturas ni obstrucciones de los anastomosis yeyuno-yeyunales. La recuperación postoperatoria del tracto gastrointestinal fue similar en los dos grupos, en lo referente a duración del drenaje nasogástrico, terapia con líquidos intravenosos y parálisis intestinal. Tampoco se observaron diferencias en la duración de la hospitalización. Murieron 10 pacientes en el postoperatorio, 7 en el grupo de la sutura y 3 en el grupo BAR, por razones no relaciondas con la enteroanastomosis; la tasa de mortalidad fue 7.9% en el grupo de sutura y 3.7% en el grupo BAR. El anillo biofragmentable representa una alternativa segura a la sutura manual para construir anastomosis en el intestino delgado.
