Summary
Experimental pulmonary embolism using Lipiodol and Lipiodol Ultrafluid as contrast mediums have been produced in 33 rats. Various stages of further development were investigated by means of light and electron microscopy.
Arteries mainly are obstructed after Lipiodol whereas after Lipiodol Ultrafluid the capillaries are subjected to embolisms. Higher viscosity of Lipiodol and thrombocyte clots lead to greater and more frequent bleedings in the lungs.
Immediately after lethal doses of Lipiodol and less numerous after Lipiodol Ultrafluid, desquamated typ II-Pneumocytes are observed in the alveolar spaces. After sublethal doses of both contrast mediums macrophages are predominating, which take up and disintegrate contrast medium.
Between the 2nd and the 16th day of the experiment, intracapillar, interstitial and intraalveolar rhomboid crystals up to 30 Μ could be found in haemorrhagic parts of the lungs. They appear after Lipiodol Ultrafluid earlier, but in smaller quantities and consist of haemoglobin or lipid.
After the 8th day granulomes of epitheloid cells developed around the interstitial deposits of the contrast medium.
Lipid deposition in the alveoles and proliferation of typ II-Pneumocytes with transformation into typ I cells were predominating between 16–20 days. This fact must be considered as morphologic base of reduced diffusion capacity, clinically observed even after weeks.
After 5 months initial state was restored after use of each of the two contrast mediums. Only some interstitial macrophages and fibrous patches were seen.
Zusammenfassung
An 33 Ratten wurde licht- und elektronenmikroskopisch der zeitliche Ablauf der Lungenveränderungen nach experimenteller Lungenembolie mit den Kontrastmitteln Lipiodol und Lipiodol Ultrafluid untersucht.
Nach Lipiodolinjektion sind vorwiegend Lungenarterien, nach Lipiodol Ultrafluid vorwiegend Capillaren embolisch verschlossen. Höhere Viscosität des Lipiodols und Thrombocytenthromben fÜhren zu größeren und zahlreicheren Parenchymblutungen.
Unmittelbar nach letalen Lipiodoldosen werden in den Alveolen desquamierte Typ II-Pneumocyten und Makrophagen beobachtet. Diese treten nach letaler Lipiodol-Ultrafluiddosis weniger zahlreich in Erscheinung. Nach subletalen Dosen beider Kontrastmittel Überwiegen Alveolarmakrophagen, die Kontrastmittel phagocytieren und abbauen.
Zwischen dem 2. und 16. Versuchstag können in hämorrhagischen Lungenabschnitten jodhaltige, intracapillär, interstitiell und intraalveolär gelegene, bis 30 Μ große rhomboide Kristalle nachgewiesen werden. Diese treten nach Lipiodol Ultrafluid deutlich frÜher, jedoch spärlicher auf. Bei den Kristallen handelt es sich um jodhaltige Fett- und Hämoglobinkristalle. Nach 8 Tagen können um interstitiell gelagerte Kontrastmitteldepots Epitheloidzellgranulome beobachtet werden.
Die morphologischen Grundlagen der klinisch noch Wochen nach Lymphographien nachweisbaren Diffusionsstörungen stehen in Zusammenhang mit intraalveolären Lipid- bzw. Kontrastmittelablagerungen sowie Proliferationen der Typ II-Pneumocyten mit Umwandlung in Typ I-Pneumocyten. Die Veränderungen der Typ II-Pneumocyten erreichten ihr Maximum zwischen dem 16. und 20. Versuchstag.
Nach 5 Monaten ist mit Ausnahme vereinzelter interstitieller Makrophagengranulome und fibröser Herde der frÜhere Zustand wieder erreicht.
