Indikation aktiver herausnehmbarer Geräte

Zusammenfassung

Nach kurzer Darstellung der historischen Entwicklung der herausnehmbaren kieferorthopädischen Behandlungsmittel werden die Vor- und Nachteile der aktiven herausnehmbaren Geräte abgehandelt. Ihre Indikation wird dadurch bestimmt, daß sie zur Durchführung der therapeutischen Aufgabe geeignet und damit erfolgversprechend, biologisch günstig wirkend und damit unschädlich sowie leicht handzuhaben und herzustellen sein sollen; sie ist weiterhin abhängig vom Gebißbefund — Milch-, Wechselgebiß oder bleibendes Gebiß — und dem Alter sowie der Kooperationsbereitschaft von Patienten und Eltern. Nicht zuletzt ist auch die Beherrschung der Behandlungsmittel dabei von Bedeutung. Aufgrund des morphologischen Befundes und der durchzuführenden therapeutischen Maßnahmen erscheint die Verwendung aktiver herausnehmbarer Behandlungsgeräte vor allen Dingen im Milch- und Wechselgebiß zur transversalen Erweiterung mit gleichzeitiger Einordnung der Frontzähne, zu alveolären Zahnbewegungen und zur Beeinflussung der Bißhöhe besonders geeignet. Unter bestimmten Voraussetzungen ist aber auch bei älteren Patienten — der Empfehlung vonNord entsprechend — noch eine Indikation für ihre Anwendung bei besonderen Aufgaben gegeben.

Summary

After a short description of the historical development of removable orthodontic applicances, their advantages and disadvantages are discussed. The advantages are that they are well suited for therapeutic treatment, require minimal skill in handling and manufacturing and are biologically safe. Their indications depend on the stage of dental development (i.e. deciduous, mixed or permanent) on the age of the patient as well as cooperation between patient and parent. Lastly of great importance is the degree of experience when using the appliance. The use of active removable appliances is especially indicated in the deciduous and mixed dentition, to expand the arch and align the front teeth. In addition, they are also very effective for moving teeth and altering the height of the bite. In certain circumstances, as suggested byNord, they can play a useful role in the treatment of adult patients.

Nous allons traiter des avantages et inconvénients des appareils amovibles actifs, en fonction du développement historique de ce type d'appareillage, en orthopédie dento-faciale. Leur indication tient principalement au fait que leur action thérapeutique mène au succès par des procédés biologiquement favorables et inoffensifs et que leur construction et maniement sont aisés. Leur emploi est aussi dépendant de la dentition (dentures temporaire, mixte ou permanente), ainsi que de l'âge et de la coopération des parents et patients. Enfin, il est tout à fait nécessaire de bien maîtriser ce moyen de traitement. Sur la base de constations morphologiques et thérapeutiques, il apparait que l'utilisation d'apperails amovibles est plus particulièrement indiquée en denture temporaire et mixte, pour effectuer des expansions transversales conjuguées avec l'alignement des dents antérieures, pour obtenir des déplacements dento-alvéolaires et pour influencer la hauteur d'occlusion. Enfin, selon la recommendation deNord, et dans des cas très particuliers, on peut encore recourir à l'action de cet appareillage chez des patients plus âgés.
