Summary
The slow upstroke velocity of the action potential of primary pacemaker cells of the sinoatrial node suggests the slow membrane channel to be involved in the excitation process of these cells. In order to prove this the effect of promotors and inhibitors of the slow membrane channel upon the excitation process of the isolated sinoatrial node of rabbits was studied.

The action potentials of primary pacemaker cells had an upstroke velocity of 1.7±1.1 V/sec and an overshoot of 8.1±4.6 mV with a threshold potential of −40.1±4.5 mV. A decrease of the extracellular Ca concentration from 2 mM to 0.2 mM led to reduction of upstroke velocity and overshoot whereas an increase from 2 mM to 4 mM augmented both parameters. But the inward current is not only carried by Ca ions but by Na ions as well, since Na withdrawal diminished upstroke velocity and overshoot.

The promotors of the slow inward current, isoproterenol and caffeine, produced a significant increase of upstroke velocity and overshoot. On the other hand, verapamil (1 mg/l) and D 600 (0.4 mg/l) completely blocked the excitation process within 20–25 min. The same effect could be produced by the bivalent cations Ni (1 mM), Co (1 mM), and Mn (1 mM). These organic and inorganic inhibitors exerted their blocking effect without significant changes of the maximal diastolic potential and the threshold potential.

In the continued presence of Ni, Co and Mn ions their inhibitory effect could be neutralized nearly completely by isoproterenol (2.5 mg/l). But excess Ca (6 mM) increased the upstroke velocity only to a small degree. In contrast to the findings in the ventricular myocardium the blocking effect of verapamil and D 600 could be overcome neither by isoproterenol nor by excess Ca.
The excitation process in the sinoatrial node is mediated solely by the slow membrane channel. It allows the influx of both Ca and Na ions which act as charge carrier of the slow inward current. The fact, that the inhibitory effect of verapamil and D 600 cannot be neutralized by catecholamines or excess Ca seems to indicate that some properties of the slow membrane channel are not exactly identical in sinoatrial pacemaker cells and in the ventricular myocardium.
Zusammenfassung
Das Aktionspotential der primären Schrittmacherzelle des Sinusknotens zeichnet sich durch eine langsame Aufstrichsgeschwindigkeit aus. Diese seit langem bekannte Tatsache deutet darauf hin, daß der Erregungsprozeß der Schrittmacherzelle des Sinusknotens durch den langsamen Membrankanal vermittelt wird. Zur weiteren Untermauerung dieser Vorstellung wurde daher am isolierten Sinusknoten-Präparat des Kaninchens der Einfluß von Inhibitoren des langsamen Einwärtsstromes sowie von Stoffen, die diesen Membranstrom vergrößern, auf den Erregungsprozeß analysiert.

Die Aufstrichsgeschwindigkeit des Schrittmacheraktionspotentials betrug durchschnittlich 1,7±1,1 V/sec. Bei einem durchschnittlichen Schwellenpotential von 40,1±4,5 mV wurde ein Overshoot von 8,1±4,6 mV gemessen. Eine Senkung der extrazellulären Ca++-Konzentration von 2 mM/l auf 0,2 mM/l hatte eine Abnahme von Aufstrichsgeschwindigkeit und Overshoot zur Folge. Umgekehrt trat nach Erhöhung der extrazellulären Ca++-Konzentration von 2 mM/l auf 4 mM/l eine Zunahme beider Parameter auf. Ca++ ist jedoch nicht das alleinige stromtragende Kation des Einwärtsstromes am Sinusknoten, da nach Senkung der extrazellulären Na+-Konzentration ebenfalls eine Verminderung von Overshoot und Aufstrichsgeschwindigkeit zutage trat.

Isoproterenol bzw. Coffein bewirkten eine Zunahme von Aufstrichsgeschwindigkeit und Overshoot des Schrittmacheraktionspotentials. D 600 (0,4 mg/l) und Verapamil (1 mg/l) hemmten dagegen den Erregungsprozeß und bewirkten nach einer Einwirkungszeit von 20–25 min einen Stillstand der Schrittmacherzelle. Der gleiche Effekt trat auch nach Zugabe von Ni++-, Co++- oder Mn++-Ionen in einer Konzentration von jeweils 1 mM/l auf. Signifikante Veränderungen des maximalen diastolischen Potentials und des Schwellenpotentials zeigten sich hingegen unter dem Einfluß dieser organischen und anorganischen Inhibitoren nicht.

Trotz weiterer Anwesenheit von Ni++-, Co++- oder Mn++-Ionen bewirkte Isoproterenol (2,5 mg/l) eine Zunahme von Aufstrichsgeschwindigkeit und Overshoot. Dadurch ließ sich bei den meisten Experimenten die Hemmwirkung dieser bivalenten Kationen auf den Erregungsprozeß nahezu vollständig beseitigen. Auch Exzeß-Ca++ (6 mM/l) steigerte in dieser Situation Aufstrichsgeschwindigkeit und Overshoot. Dieser Effekt war jedoch relativ gering ausgeprägt und hatte in keinem Falle eine Neutralisierung der Ni++-, Co++- und Mn++-Hemmwirkung zur Folge. Im Gegensatz zum Arbeitsmyokard gelang es jedoch nicht, eine durch Verapamil oder D 600 induzierte Hemmung des langsamen Einwärtsstromes durch Exzeß-Ca++ oder Katecholamine wieder aufzuheben.
Die Befunde zeigen, daß der Erregungsprozeß der Sinusknotenzelle hauptsächlich durch den langsamen Membrankanal vermittelt wird. Dieser Membrankanal erlaubt infolge seiner Kationen-Unspezifität den Influx sowohl von Ca++ als auch von Na+. Beide Kationen können daher als Ladungsträger des langsamen Einwärtsstromes während der Erregung der Sinusknotenzelle fungieren. Die Tatsache, daß im Gegensatz zum Arbeitsmyokard die blokkierende Wirkung von Verapamil und D 600 durch Exzeß-Ca++ oder Katecholamine nicht beseitigt werden kann, deutet darauf hin, daß einige Eigenschaften des langsamen Membrankanals am Sinusknoten und an der Arbeitsmyokardzelle nicht völlig identisch sind.
