95. Gibt es Fortschritte bei der operativen Behandlung der Sigmadiverticulitis?

Summary

A high percentage of people in the civilized Western world develops colonic diverticula. Signs of illness are coming from inflammation around the diverticula. Resection therapy is indicated after several inflammation episodes. The operation has often to be performed in case of emergency: Ileus, bleeding, perforation. We recommend the primary resection of the sigmoid colon with an transverse colostomy or the Hartmann operation even in case of perforation or peritonitis. We operated 140 patients. The mortality rate in our hospital was 12% of the elective and 27% of the emergency operations.

Zusammenfassung

Die Divertikulose kommt bei der Bevölkerung hochzivilisierter Industrienationen vor. Beschwerden treten nur dann auf, wenn es zu einer Diverticulitis kommt. Die Behandlung ist primär konservativ. Die Indikation zur Elektivoperation ergibt sich nach mehreren Entzündungsschüben. Bei Komplikation wie Ileus, Blutung, Fistelbildung oder Perforation wird notfallmaBig operiert. Empfohlen wird dann die Sigmaresektion mit Quercolonafter oder das Verfahren nach Hartmann. Im eigenen Krankengut von 140 Patienten war die perioperative Letalität 12% bei Elektiveingriffen und 27% bei Notfalleingriffen.
