86. Operationstaktik und Ergebnisse bei erweiterten Operationen colorectaler Carcinome

Summary

Between 1969 and 1983 a total of 1918 patients with colorectal carcinoma were curatively operated. In 121 of these patients a curative procedure could be achieved despite tumor invasion of adjacent organs by extending the operation to include these involved structures. In 45% of these patients the histological examination revealed that there were only peritumoröus inflammatory adhesions while in 55% of the cases a direct tumor invasion of the neighbouring organ was present. The 5-year-survival-rate in the 121 patients was 44%. The operative mortality of 12% (14 out of 121) is included. The final results were considerably worse if the tumor could not be removed en bloc.

Zusammenfassung

Zwischen 1969 und 1983 wurden 1918 Patienten mit colorectalen Carcinomen kurativ operiert. Unter diesen wurde bei 121 Patienten wegen makroskopischen Tumorbefalls von Nachbarorganen eine erweiterte Resektion vorgenommen. Bei der pathohistologischen Begutachtung der Präparate zeigte sich, daß in 55% die mitentfernten Organe tatsächlich tumorbefallen waren, während bei 45% der Patienten nur peritumoröse entzündliche Adhaesionen vorlagen. Die 5-Jahres-Überlebensrate der 121 Patienten betrug 44%. Die postoperative Letalität von 12% (14 von 121) ist eingeschlossen. Die Heilergebnisse waren deutlich schlechter, wenn der Tumor nicht en bloc entfernt werden konnte.
