Abstract
This article describes a method for reclassifying causes of death in the Netherlands for the period 1875–1992. Two criteria should be met to obtain a useful classification to study the epidemiological transition. First, the categories should be nosologically continuous over the period under study and second, there should be enough detail in causes which are important in the context of the epidemiological transition viz. communicable diseases, non-communicable diseases and external causes of death. A method developed by Vallin and Meslé (1988), which involves ‘dual correspondence tables’ and ‘fundamental associations’, was used to create nosologically continuous categories. These categories were tested for statistical continuity during the transition years of one ICD-revision to the next, using ordinary least squares regression analysis. The reclassification procedure resulted in a nested classification consisting of three levels of refinement of causes of death: 27 causes, 1875–1992; 65 causes, 1901–1992; and 92 causes, 1931–1992. On the basis of this classification, 43% of all deaths in 1875–79 and 98% of all deaths in 1992 could be allocated to either communicable diseases, non-communicable diseases or external causes.
Résumé
Cet article présente une méthode de reclassement des décès par cause en Hollande pour la période 1875–1992. En vue d'obtenir une classification utile pour étudier la transition épidémiologique, il faut répondre à deux critères. D'abord, les catégories doivent fournir une classification continue tout au long de la période étudiée. Ensuite, les causes qui sont importantes dans le contexte de la transition épidémiologique (plus précisément, qui permettent de distinguer entre maladies transmissibles, maladies non transmissibles et causes externes de décès), doivent être suffisamment détaillées. On a utilisé ici une méthode mise au point par Vallin et Meslé (1988), qui établit des «tables de correspondance selon deux axes» et des «associations de rubriques», en vue de créer des catégories nosologiques suivies. La continuité statistique de ces catégories a été testée pendant les années de transition, d'une révision de la Classification Internationale des Maladies (C.I.M.) à la suivante, à l'aide de régressions utilisant la méthode des moindres carrés. La procédure de reconstitution conduit à une classification à trois niveaux emboîtés selon la précision des causes de décès: 27 causes de 1875 à 1992, 65 causes de 1901 à 1992, et 92 causes de 1931 à 1992. Sur la base de cette classification, 43% des décès en 1875–79 et 98% en 1992 ont pu être affectés soit aux maladies transmissibles, soit aux maladies non transmissibles, soit enfin aux causes externes.
