Summary
Subphrenic abscesses have, against all expectations, increased rather than decreased in number. In the suppurative processes in the Subphrenic region the masking effect of antibiotics, the reduced immunity response due to cortico-steroids, the polypragmatic diagnostic approach and the shallowness of general surgical training all play an equal, sometimes competing, sometimes overlapping part.
It is worth mentioning that in contrast to earlier in-vestigations, cholelithogenic infections nowadays are aetiologically more often the cause than the previously predominant diseases of the bowel and particularly appendicitis. A considerable part is played in the causation of these infections be the multiple traumas sustained in road accidents, also by tropical germs and parasites. Symptoms and diagnosis are often made obscure, veiled and atypical. Rational treatment is hereby delayed and its effectiveness reduced.
A timely surgical intervention remains, as ever, the method of choice. The various forms of Subphrenic abscesses, which frequently affect both sides are discussed on the basis of our own clinical material and appropriate therapeutic guide lines recommended.
Zusammenfassung
Die Eiterungen des Subphreniums haben entgegen allen Erwartungen in ihrer Frequenz eher zu- als abgenommen. Bei den Eiterungen des subphrenischen Raumes spielen Verschleierungstendenzen durch Antibiotics, Herabsetzung der Immunitätslage durch Corticosteroide, polypragmatische diagnostische Handlungen, eine Verflachung der allgemeinen chirurgischen Ausbildung eine fast gleichwertige, miteinander konkurrierende bzw. sich iiberschneidende Rolle. ]Bemerkenswert 1st ferner, da\sB im Gegensatz zu \:alteren Untersuchungen \:atiologiseh die cholelithogenen Infektionen jetzt h\:aufiger anfallen als die friiher urs\:achlich dominierenden Darmerkrankungen and insbesondere die Appendicitis. Eine erhebliche Rolle als Infektionsursache spielen auch die Auswirkungen von Massentraumen der Verkehrsunf\:alle Bowie tropische Erregerarten and Parasiten. Die Symptomatik and damit auch die Diagnostik ist heute hierdurch vielfach kaschiert, verschleiert and atypisch. Die sachgerechte Behandlung wird hierdurch verz\:ogert and in ihrer Effizienz gemindert.
Nach wie vor ist die rechtzeitige chirurgische Intervention die Methode der Wahl. Die verschiedenen Formen der Eiterungen des Subphreniums, das häufig beidseitig befallen sein kann, werden anhand der eigenen Kasuistik diskutiert and entsprechende therapeutische Richtlinien empfohlen.
