9. Dringliche plastische Versorgung großer Weichteildefekte

Summary

With soft-tissue lesions, debridement and skin coverage are mandatory and a precondition for the repair of damage to tendons, nerves, vessels and bones, as well as for the prevention of infection. When the condition of the patient permits and facilities and personnel are available, the primary treatment should include the reconstruction of form and function, because secondary reconstructive procedures usually entail additional problems. In a casualty service it is necessary to have a fully trained and equipped plastic surgery team, also a skin bank, to ensure optimal planning and performance of primary reconstructive measures involving the use of large flaps and grafts.

Zusammenfassung

Bei der Versorgung eines Weichteildefektes sind das Débridement und die Deckung mit Haut unaufschiebbar, und eine Voraussetzung für Sehnen-, Nerven-, Gefäß- oder Knochennähte, sowie für die Verhütung der Infektion. Wenn der Zustand des Patienten, sowie die lokalen und personellen Verhältnisse dies erlauben, soll die Wiederherstellung der Form und Funktion primär erfolgen, da bei sekundärer Rekonstruktion sich in der Regel zusätzliche Schwierigkeiten ergeben. Für die optimale Planung und Durchführung primär rekonstruktiver Verfahren, die Verwendung großer Transpositionslappen, abgetrennter Weichteile, Spalthauttransplantate oder Fremdhaut ist bei der Versorgung von Schwerverletzten ein entsprechend ausgebildeter und ausgerüsteter plastisch-chirurgischer Dienst, sowie eine Hautbank notwendig.
