3. Dringlichkeitsfragen bei der Erstversorgung kombinierter und Mehrfachverletzungen

Summary

Combined injuries for the purposes of this lecture are simultaneous injuries in different regions of the body, body cavities or organs, of which only one or the combined effect of several lead to a state of emergency, that is to an acute threat to the life of the injured. An analysis of the localization of such injuries and their combination in 292 persons injured in this way, demonstrates the multitude of possibilities and mentions a few typical combinations.

In an attempt to arrive at a systematic diagnostic and therapeutic procedure a stepwise plan is worked out. The first step is solely concerned with assessing the state of the vital systems, that is the central nervous system, respiration and circulation. A threatening haemorrhage can already call for an emergency operation at this stage. The second step involves extensive diagnostic procedures which are necessary to clarify the indications for early surgery (trephination, thoracotomy, laparotomy).

The third step commences only after circulation and respiration function normally. There is then time to plan carefully the necessary surgical measures as for instance the treatment of injuries of the extremities.

Zusammenfassung

Kombinationsverletzungen im Sinne des Themas sind gleichzeitige Verletzungen verschiedener Körperregionen, Körperhöhlen oder Organe, von denen eine allein oder die summarische Auswirkung mehrerer zur Notfallsituation, d.h. zur akuten Bedrohung des Lebens des Verletzten führt. Eine Analyse der Verletzungslokalisationen und -kombinationen von 292 derartig Verletzter demonstriert die Vielfalt der Möglichkeiten und stellt einige typische Kombinationsformen heraus. Dem Versuch zu einem systematisierten diagnostischen und therapeutischen Vorgehen wird ein Stufenplan zugrunde gelegt. Die erste Stufe gilt allein der Erfassung des Zustandes der Vitalsysteme, Gehirn — Atmung — Kreislauf. Eine bedrohliche Blutung kann in dieser Stufe bereits den Noteingriff erfordern. In der 2. Stufe ist eine ausführlichere Diagnostik möglich und notwendig, insbesondere zur Klärung der Frage einer Indikation zur Frühoperation (Trepanation, Thorakotomie, Laparotomie). Die 3. Stufe beginnt erst nach Normalisierung von Zirkulation und Respiration. Sie bietet Raum zu planmäßig vorbereiteten operativen Eingriffen, beispielsweise zur Behandlung von Extremitätenverletzungen.
