Zusammenfassung
Blutspiegelbestimmungen von Meprobamat bei Patienten mit Lebercirrhose und progredienter chronischer Hepatitis ergeben einen verzögerten Abfall gegenüber leber- und nierengesunden Patienten. Die Halbwertszeiten betragen bei den Kranken mit chronischen Leberschäden 24,3±4,4 Std, bei den lebergesunden Probanden 12,6±2,5 Std. Nach 10tägiger Behandlung mit 0,2 g Phenobarbital/die kommt es zu keiner Veränderung der Halbwertszeiten des Meprobamat im Blut. Dagegen sind die Halbwertszeiten des Meprobamat bei 2 Patienten mit Arzneimittelsucht auf 4,5 bzw. 7,0 Std verkürzt. Die Befunde zeigen erstmals auf, daß bei chronischen Leberschäden mit einer signifikant veränderten Pharmakokinetik von Arzneimitteln gerechnet werden muß.
Summary
Patients with liver cirrhosis and chronic hepatitis have a prolonged half-life of meprobamate in the blood compared with persons without liver or kidney damage. The half-life in patients with chronic liver diseases is in the mean 24,3±4.4 hours, the half-life in patients without liver disease is 12.6±2.5 hours. After a 10 day treatment with 0.2 g Phenobarbital/die there is no alteration of the half-life of meprobamate in the blood. On the contrary to this observation the half-life in 2 patients with severe drug addiction is shortened to 4.5 respectively 7.0 hours. The results firstly show that in chronic liver diseases the pharmacocinetic of drugs may be altered.
