Summary
Disregarding intestinal occlusion and ischemia, peritonitis without perforation is rare. Primary peritonitis is caused by infection of intraperitoneal bile, pancreatic juice, chyle or blood. A separate entity is spontaneous bacterial peritonitis (SBP) in cirrhotic patients with ascites. Hematogenous metastatic peritonitis occurs in early childhood. Exogenous infection of the peritoneal cavity is rare. Gynecological infections tend to involve the abdomen more often than urological. Postoperative peritonitis without perforation is caused by exogenous, endogenous or preexistent infections. Laparotomy is indicated for evacuation of secretions, drainage and prophylaxis of ileus.
Zusammenfassung
Mit Ausnahme der Durchwanderungsperitonitis ist die perforationslose Peritonitis selten. Ursachen der primären Peritonitis sind Infektionen von Galle, Pankreassaft, Chylus oder Blut. Ein eigenes Krankheitsbild ist die spontane bakterielle Peritonitis (SBP) des Lebercirrhotikers mit Ascites. Hämatogen-metastatische Peritonitiden bevorzugen das Kindesalter. Exogene Infektionen sind selten. Gynäkologische Infektionen greifen eher auf die Bauchhöhle über als urologische. Die postoperative perforationslose Peritonitis ist Folge einer exogenen, endogenen oder fortbestehenden Infektion. Ziele der Laparotomie sind Entleerung des Sekrets, Drainage und Ileusprophylaxe.
