Zusammenfassung
Die enterale Resorption und die renale Ausscheidung von Calcium ist bei chronischer Niereninsuffizienz deutlich vermindert. Die Calciumausscheidung in den Darm ist normal, durch mangelhafte Rückresorption des hier ausgeschiedenen Minerals ist eine chronisch-negative Calciumbilanz zu erklären. Wichtig ist auch die Feststellung, daß Urämiker häufig calciumarm ernährt werden.
Wir fanden eine normale oder leicht erhöhte Knochenanbaurate im Calcium-47-Test bei 5 Patienten. In Übereinstimmung mit den histologischen Befunden ergibt sich das Bild einer Osteopathie mit lebhaftem Umsatz. Trotzdem kann offenbar ein normaler Calciumspiegel im Serum nicht aufrechterhalten werden. Verminderte Calciumreserven mögen bei der Pathogenese der Hypocalcämie eine Rolle spielen, vorwiegend handelt es sich aber um eine Regulationsstörung, denn wir finden herabgesetzte Calciumspiegel im Serum auch bei vergrößertem mobilen Calciumpool und bei akuter Niereninsuffizienz. Wahrscheinlich stellen Resorptionsstörungen und Hypocalcämie Schutzmechanismen gegen eine Überflutung des Organismus mit Calcium und extraessären Verkalkungen dar.
Summary
Enteral absorption and renal excretion of calcium are markedly decreased in chronic renal failure. Excretion of calcium into the gut is normal, and by impaired re-absorption of endogenous enteral calcium a long-standing negative calcium-balance can be produced. One also should bear in mind that uremic patients frequently eat a low-calcium diet.
We studied the calcium-balance and the47-calcium turnover in 5 unselected cases of chronic renal failure. A rather high bone-turnover (with highly normal or raised bone-accretion-rate) is seen in accordance with the histological findings. In spite of this a normal serum calcium can not be maintained. Diminished calcium-stores of the skeleton may be involved in the pathogenesis of this hypocalcaemia but it is felt that there is mainly a kybernetic disorder of blood-bone equilibrium since low serum calcium is also encountered in the presence of a large calcium-pool and in acute renal failure. Hypocalcaemia and decreased absorption probably protect the organism from extraosseous calcification.
