Zusammenfassung
Die renale Ausscheidung der Glutaminsäure-Oxalessigsäure-, Glutaminsäure-Brenztraubensäure-Transaminase, Leucin-Aminopeptidase, β-Glucuronidase und alkalischen Phosphatase wurde bei 30 Kindern mit nephrotischem Syndrom insgesamt 72mal bestimmt. Eine signifikant gesteigerte Enzymurie wurde im Vergleich mit der Kontrollgruppe nur bei Fällen mit einer Eiweißausscheidung von über 1 g/die beobachtet.
Die Transaminaseausscheidung wird vom jeweiligen Zustand der Nieren bestimmt: Die höchste Elimination war in den schweren, therapieresistenten Fällen vorhanden. Die hochgradige Nierenläsion ist keine Vorbedingung der extrem hohen β-Glucuronidase- und Leucin-Aminopeptidaseausscheidung, eine solche wurde auch in benignen Fällen während des Rezidivs beobachtet.
Aus den Berechnungen ging hervor, daß in der akuten Phase des nephrotischen Syndroms der renale Enzymverlust die Bewertung der Serumaktivität bei der β-Glucuronidase und Leucin-Aminopeptidase verfälschen kann, die Serumaktivität der ubrigen Enzyme jedoch nicht in entscheidender Weise zu beeinflussen vermag.
Summary
In 30 children with nephrotic syndrome renal excretion of glutamic-oxalacetic-, glutamic-pyruvic-transaminase, leucine aminopeptidase, β-glucuronidase and alkaline phosphatase was determined 72 times. A significant increase of enzymuria as compared to the control group was observed only in cases having a proteinuria of more than 1 g/day.
Transaminase excretion depends on the conditions of the kidneys: elimination was highest in infaust cases. Extremely high β-glucuronidase and leucine aminopeptidase excretion is not necessarily due to severe renal lesion, it was also observed in milder cases during recidivations.
The calculations showed that during the acute phase of the nephrotic syndrome the evaluation of the serum activity of β-glucuronidase and leucine aminopeptidase may be completely misleading because of the renal enzyme loss which, however, has evidently no significant effect on the serum activity of the other enzymes.
