Summary
An indirect test for exocrine pancreatic function was performed with 206 subjects using N-Benzoyl-L-tyrosyl-paraaminobenzoic acid (N-BT-PABA), a synthetic tripeptide specific for chymotrypsine in conjunction with a PABA-resorption test using an equimolar dose of p-aminobenzoic acid (PABA). The amount of PABA excreted in the urine over 9 h after enzymatic enteral hydrolysis of N-BT-PABA and resorption of PABA was taken as a measure of exocrine pancreatic function if normal excretion rates could be proved in the resorption-test. This applied, however, in merely about 80 percent of the subjects. Ever since the PABA resorption test is being performed by administering a 1 gramme dose of pancreatine simultaneously with the peptide the test is always evaluable. Technical failures can be due to insufficient amounts of urine and interference of numerous drugs with the analytical laboratory method. The test has been performed in two modifications (Stimulation of the pancreas with Lundh's test meal or with an amino acid mixture).
Healthy subjects (n=68,n=21) could be significantly distinguished (p<0.001) from patients with definite pancreatopathy (n=51,n=8) in either modification of the test with merely small overlap of the groups. The results of the peptide test are in significant correlation (p<0.01;n=82) with the titrimetric method for chymotrypsin in the feces. The stomach passage of the stimulating test meals has been studied with99mTc-DPTA. There existed no correlation between percent of PABA excretion per dose within 9 h and the stomach passage half-time of the test meals. The stimulating effect of amino acid mixture on exocrine pancreas was higher than that of Lundh's meal.
Notwithstanding the possibilities of technical failure the peptide-PABA test is a valuable test for exocrine pancreatic function.
Zusammenfassung
Mit dem synthetischen, chymotrypsinspezifischen Tripeptid N-Benzoyl-L-Tyrosyl-PABA (PABA = p-Aminobenzoesäure) wurde ein indirekter Pankreasfunktionstest zusammen mit einem PABA-Resorptionstest (äquimol. Dosis PABA) bei 206 Probanden durchgeführt. Die im 9 h-Sammelurin nach enzymatischer Spaltung und Resorption ausgeschiedene Menge PABA galt als Maß für die exokrine Pankreasfunktion, wenn im Resorptionstest eine normale PABA-Resorption und Exkretion nachgewiesen war. Dies war jedoch nur bei etwa 80% der Probanden der Fall. Seitdem der PABA-Stoffwechsel durch Gabe von Peptid mit 1 g Pankreatin geprüft wird, ist der Peptid-Test immer auswertbar. Technische Fehlermöglichkeiten liegen in zu geringen Sammelurinmengen und in der Störanfälligkeit der analytischen Methode durch zahlreiche Arzneimittel. Der Test wurde in 2 Modifikationen (Pankreasstimulation mit Lundh-Testmahl und mit Aminosäuregemisch) durchgeführt.
Pankreasgesunde (n=68,n=21) ließen sich in beiden Testmodifikationen von Patienten mit gesicherter Pankreasinsuffizienz (n=51,n=8), bei nur geringer Überlappung im Grenzbereich, signifikant unterscheiden (p<0,001).
Eine Korrelation der Ergebnisse des Peptid-Tests mit der titrimetrischen Stuhlchymotrypsinmethode war signifikant (p<0,01 (n=82)). Mit99mTc-DPTA markierten, beim Peptidtest zur Pankreasstimulation verwendeten Testmahlzeiten (Aminosäuregemisch und Lundh-Mahl) wurde die Halbwertszeit der Magenentleerung (MEZ 1/2) geprüft. Es fand sich eine, wenn auch nicht signifikant höhere prozentuale PABA-Exkretion p.d. und 9 h mit dem Aminosäuregemisch. Eine Korrelation zur MEZ 1/2 bestand nicht. — Unter Berücksichtigung der Fehlermöglichkeiten eignet sich der Peptid-PABA-Test als Suchmethode für exokrine Pankreasinsuffizienz.
