Abstract
Seventy-three patients underwent total colectomy, rectal mucosectomy, creation of J or S ileal reservoir, and ileal pouch-anal anastomosis from 1982 to 1989. Mean follow-up was 38 months, with a minimum of 3 months in 15 patients being followed long-term at another institution. Forty-eight (66 %) patients had histologically proven ulcerative colitis and 25 (34%) patients had familial polyposis. Thirty-eight J reservoirs and 35 S reservoirs were constructed. There were no perioperative deaths. The failure rate (loss of pouch) was 3%. Thirty-six complications in 34 (47%) patients were reported, 14 (19%) patients required surgery. Bowel obstruction was the most common postoperative complication (16%), followed by pouchitis (15%), and cuff infection (5%). Seventy-eight percent of the complications were associated with the J pouch. Average stool frequency at 1 year was 4 per 24-hour period. Other complications included postoperative pneumonia (1), peroneal nerve palsy (1), and temporary sexual dysfunction (1). Seven of 15 complications requiring surgical intervention occurred in the first 2 years of the study period, illustrating the learning curve associated with the procedure. Blood loss, transfusion requirements, and length of operation were not associated with higher complication rates. Use of the J pouch and experience of the individual surgeon affected morbidity.
Résumé
Soixante-treize patients ont eu une colectomie totale, une mucosectomie rectale, avec création d'un résevoir iléal en “J” ou en “S” et suivie d'une anastomose iléo-anale entre 1982 et 1989. La durée moyenne du suivi a été de 38 mois. Pour 15 patients suivis dans un autre établissement, le suivi postopératoire n'a été que de 3 mois. Quarante-huit patients (66%) avaient une rectocolite hémorragique prouvée histologiquement; 25 (34%) avaient une polypose familiale. Il y avait 38 résevoirs en “J” et 35 en “S”. Il n'y avait aucune mortalité périopératpire. Le taux d'échec (ablation de résevoir) était de 3%. Trente-six complications ont été observées chez 34 patients (47%); 14 (19%) ont nécessité un traitement chirurgical. L'occlusion intestinale a été la complication post-opératoire la plus fréquente (16%), suivie de pouchite (15%) et d'infection périanastomotique (5%). Soixante-huit pour cent des complications sont survenues sur résevoir en “J”. Toutes techniques confondues, la fréquence moyenne des selles à un an était de 4/24 heures. D'autres complications ont été observées: pneumopathie post-opératire (n=1), paralysie du nerf péronier (n=1), troubles sexuels transitoires (n=1). Sept des 15 complications nécessitant une ré-intervention chirurgicale sont survenue dans les deux premières années de notre expérience, ce qui montre qu'il existe une courbe d'apprentissage avec cette technique. Les pertes sanguines, les besoins de transfusions et la durée de l'intervention n'étaient pas corrélées avec un taux élevé de complications. L'utilisation d'un résevoir en “J” et le manque d'expérience du chirurgien étaient directement liés à la morbidité.
Resumen
Setenta y tres pacientes fueron sometidos a colectomía total, mucosectomía rectal, creación de un reservorio ileal en J o en S y anastomosis de la boisa ileal al ano, en el periódo 1982 a 1989. El promedio de seguimiento fue de 38 meses, con un mínimo de 3 meses en 15 pacientes seguidos a largo plazo en otra institutión. Cuarenta y ocho pacientes (66%) tenían colitis ulcerativa histológicamente comprobada y 25 (34%) tenfan poliposis familiar. Se construyeron 38 reservorios en J y 35 en S; no se presentaron muertes perioperatorias. La tasa de falla (pérdida de la boisa) fue de 3%. Se informaron 36 complicaciones en 34 pacientes (47%), 14 (19%) de las cuales requirieron cirugía. La obstruction intestinal fue la complicación postoperatoria más comÚn (16%), seguida de “bolsitis” (15%) e infection local (5%). 78% de las complicaciones aparecieron asociadas con la boisa en J. El promedio de deposiciones al cabo de 1 año fue de 4 por 24 horas. Otras complicaciones incluyeron neumonía postoperatoria (1), parálisis de nervio peroneal (1) y disfunción sexual temporal (1). Siete de 15 complicaciones que requirieron interventión quirÚrgica ocurrieron en los primeros 2 años de nuestra experiencia, lo cual ilustra la curva de aprendizaje que esta asociada con este procedimiento. La pérdida de sangre, los requerimientos de transfusion y la duratión de la operatión no se vieron asociados con mayores tasas de complicatión. El uso de la boisa en J y la experiencia del cirujano individual son factores que afectan la morbilidad.
