New techniques in hernia repair

Abstract

The surgical treatment of inguinal and femoral hernias has evolved through several stages to reach a modern and successful era. This success was shared by all our predecessors who contributed some knowledge with respect to anatomy, physiology, and surgery. Although most patients are treated with satisfaction by primary herniorrhaphy, difficult recurrences can also be treated successfully. The special features that characterize the modern treatment of difficult hernias are the preperitoneal approach and the availability of reliable prosthetic materials. Some of the techniques currently available are discussed. They are the femoral umbrella, transvérsalis fascial replacement, and the fletching. The results have been encouraging. As surgeons, we are behooved to look, assess, and reconsider in order to improve.

Résumé

Le traitement chirurgical des hernies, inguinale et crurale, a évolué et atteint à ce jour une certaine réussite. Cette réussite, nous la devons à nos prédécesseurs qui y ont tous contribué en anatomie, en physiologie, et en chirurgie. Bien que la plupart des patients traités avec succès soient ceux qui bénéficient d'une réparation primitive, on connaît également des réussites dans la réparation des récidives. Celles-ci sont dues aux réparations faites avec des prothèses posées par voie prépéritonéales. Les techniques actuelles sont discutées: le parapluie crural, le replacement du fascia, et l'empennage prothétique. Les résultats sont encourageants. En tant que chirurgiens, nous sommes obligés de tout observer, évaluer et mettre en cause pour aboutir au progrès.

Resumen

El tratamiento quirúrgico de las hernias inguinales y femorales ha evolucionado a través de varias etapas para alcanzar la actual moderna y exitosa era. El presente éxito es compartido por todos nuestros predecesores, quienes han contribuido conocimientos relativos a la anatomía, fisiología, y cirugía de la hernia inguinal. Aunque la mayoría de los pacientes pueden ser tratados satisfactoriamente mediante herniorrafia primaria, casos recurrentes difíciles también pueden ser exitososamente tratados. Las características especiales que tipifican el tratamiento moderno de las hernias difíciles son el abordaje preperitoneal y la disponibilidad de materiales protésicos. Se revisan algunas de las técnicas quirúrgicas actualmente disponibles. Estas son la sombrilla femoral, el reemplazo de la fascia transversalis, y la técnica de fletching. Los resultados han sido prometedores. Como cirujanos estamos obligados a observar, valorar, y reconsiderar las diferentes opciones, si queremos progresar.
