Abstract
The Canadian hernioplasty is the modern equivalent of the original Bassini operation. Most of the criticism of the repair is unjustified, even the fact that the repair does not address the femoral canal. The incidence of the femoral type of recurrence that typically occurs in the first year was only 0.14% among the 4,366 primary Canadian hernioplasties performed by one surgeon in the years 1970 to 1987. There were a total of 58 recurrences for a raw recurrence rate of 1.3%. About half of the recurrences were considered to be failures of the hernioplasty, and the remaining ones were attributable to continued deterioration of the patient's tissues. This huge experience of one surgeon demonstrates that recurrence rates, to a great extent, depend on the skills of the surgeon. Recurrence rates decrease as a surgeon's experience with the procedure increases. It also demonstrates the importance of technique and how a minor change in the method of beginning the continuous suture reduced suture tension and eliminated the well-known pubic tubercle type of recurrence seen after all hernioplasties. The recurrence rate for 639 recurrent and rerecurrent hernias repaired by Canadian hernioplasty was 7.2%. This rate is unsatisfactory, and a preperitoneal repair with a polyester prosthesis is preferred for these difficult problems. A local agent neutralized with sodium bicarbonate is the anesthetic of choice, and simultaneous left and right hernioplasties with the patient going home the same day as the procedure are now commonplace.

La réparation herniaire canadienne peut être considérée comme l'équivalent moderne de l'opération de Bassini. La plupart des critiques ne sont pas justifiées, pas même celles qui soutiennent que cette technique ne convient pas aux hernies crurales. Le taux de récidive de type crural à un an était de seulement 0.14% parmi les 4,366 réparations herniaires faites par un seul chirurgien de 1970 à 1987. Il y avait un total de 58 récidives pour un taux de récidives brut de 1.3%. Environ la moitié des récidives sont des échecs de la technique, l'autre moitié pouvant être attribuée à une détérioration continue des tissus du patient. L'expérience énorme d'un seul chirurgien montre bien que le taux de récidives dépend des dons de celui-ci. Le taux de récidive décroît avec l'expérience du chirurgien. Les résultats prouvent aussi l'importance de la techique et démontrent qu'un changement mineur dans la méthode—l'utilisation d'un surjet—a réduit la tension et a éliminé la récidive au niveau de l'épine pubienne bien connue après la réparation de tout type. Le taux de récidives pour cure de hernie récidivée (le ou 2e récidive) réparées avec cette technique était de 7.2%. Ce taux n'est pas suffisant; aussi les auteurs préfèrent dans ces cas-là une réparation par prothèse prépéritonéale en polyester. On préconise de plus en plus une anesthésie locale neutralisée par du bicarbonate de sodium et la cure bilatérale avec retour au domicile le jour même.

La hernioplastia canadiense es el equivalente moderno de la operación original de Bassini. La mayor parte del criticismo a esta reparación es injustificado, aún el hecho de que la reparación no involucra al canal femoral. La incidencia del tipo femoral de recurrencia, que típicamente ocurre en el primer año, fue de sólo 0.14% en las 4,366 hernioplastias canadienses realizadas por un solo cirujano en los años 1970 a 1987. Se presentó un total de 8 recurrencias para una tasa cruda de recurrencias de 1.3%. Alrededor de la mitad de las recurrencias fueron consideradas como fallas de la hernioplastia, en tanto que el resto fue atribuible a deterioro continuado de los tejidos del paciente. Esta enorme experiencia de un solo cirujano demuestra que las tasas de recurrencia, en alto grado, dependen de la habilidad y destrezas del cirujano. Las tasas de recurrencia disminuyen en la medida que se incrementa la experiencia del cirujano con el procedimiento. También demuestra la importancia de la técnica quirúrgica y como un cambio mínimo en la manera de comenzar la sutura continua, reduce la tensión, y elimina el bien conocido tipo de recurrencia sobre el tubérculo púbico que se observa en todo tipo de hernioplastias. La tasa de recurrencia para 639 hernias recurrentes y rerecurrentes reparadas mediante la hernioplastia canadiense fue 7.2%. Esta tasa es insatisfactoria y la reparación prepreritoneal con una prótesis de poliester es preferida para el manejo de estos difíciles problemas. Un agente local mezclado con bicarbonato de sodio es el anestésico de elección; reparaciones simultáneas, derecha e izquierda, con el paciente siendo dado de alta el mismo día, representan actualmente el procedimiento veloz.
