Ultrasonic examination during surgery for abdominal abscess

Abstract

Real-time B-mode high resolution ultrasound scanning was employed during 12 operations for abdominal abscess. These included: 5 hepatic, 2 splenic, 2 pancreatic, 2 retroperitoneal, and 1 intraperitoneal abscesses. In most instances, operative ultrasonography was considered to be helpful in operative management of abdominal abscesses. Ultrasound was valuable in: (a) localizing abscesses before tissue dissection; (b) establishing the diagnosis which had not been made preoperatively; (c) excluding the presence of abscesses suspected on preoperative studies or at operative exploration; (d) identifying adjacent structures such as blood vessels, biliary and pancreatic ducts, and intestines; and (e) guiding the needle for aspiration of abscess. Operative ultrasonography is a safe and simple technique, providing the surgeon with accurate information during surgery which may reduce the re-drainage and mortality rates of abdominal abscess.

Résumé

L'échographie en temps réel a été employée au cours de 12 interventions pour abcès abdominaux, cette série comprenant des abcès du foie: 5, de la rate: 2, du pancréas: 2, de la région rétropéritonéale: 2, de la cavité péritonéale: 1.

Le plus souvent, cette exploration préopératoire a été utile pour traiter les abcès.

Elle a permis:

  1. 1)

    de localiser l'abcès avant toute dissection,

  2. 2)

    de porter le diagnostic lorsque l'abcès n'avait pas été reconnu avant l'intervention,

  3. 3)

    d'exclure la présence d'abcès qui avait été suspecté à tord avant ou au cours de l'intervention opératoire,

  4. 4)

    d'identifier les structures anatomiques adjacentes: vaisseaux sanguins, canaux biliaires et pancréatiques, intestin,

  5. 5)

    de guider l'aiguille de ponction à évacuer l'abcès.

L'échotomographie opératoire constitue une méthode simple et dénuée de danger. Elle fournit au chirurgien une information précise qui permet de réduire le taux des drainages répétés et de réduire la mortalité des abcès de l'abdomen.

Abstracto

Se empleo escanografía ultrasónica de tiempo real y de modo-B en el curso de 12 operaciones para abscesos abdominales. Estos incluyeron: 5 abscesos hepáticos, 2 esplénicos, 2 pancreáticos, 2 retroperitoneales y 1 intraperitoneal. En la mayoría de los casos se consideró que la ultrasonografía operatoria fue Útil en el manejo operatorio de los abscesos abdominales. El ultrasonido fue de valor en: 1) la localización de los abscesos antes de la disección de tejidos; 2) el establecimiento de un diagnóstico que no había sido hecho preoperatoriamente; 3) la exclusión de la presencia de abscesos que fueron sospechados en estudios preoperatorios o en el curso de una operacion exploratoria; 4) la identificatión de estructuras adyacentes tales como vasos sanguíneos, canales biliares y pancreáticos, e intestinos; y 5) guiar la aguja para la aspiración de abscesos. La ultrasonografía operatoria es una técnica segura y simple que provee al cirujano information precisa en el curso de la cirugía, la cual puede reducir la tasa de drenajes repetidos y la mortalidad de los abscesos abdominales.
