Abstract
In analyzing our observations of 115 patients with Buerger's disease during a recent 4-year period, the following results were obtained. First, Buerger's disease is primarily a disease of the small- and medium-sized vessels of the extremity. The disease commences peripherally and reaches the infrapopliteal or forearm arteries within a short time after the onset of symptoms. The majority of the patients have 3- or 4-limb involvement. Although the first ischemic manifestation is variable, a trophic lesion is the chief symptom at the first examination.
Second, in contrast to an incessant deterioration of the peripheral circulation, proximal extension is less frequent. The skip lesion plays an important role in proximal progression of the obstructive lesion in the main artery.
Third, although direct arterial surgery is infrequently feasible because of the distal occlusive process, successful arterial revascularization is most effective for healing of ulceration. All efforts should be concentrated on limb salvage.
Furthermore, microabscess consisting of giant cells, epithelioid cells, and leukocytes lends the most characteristic appearance to the thrombotic lesion at the early stage. General architecture of the vessel wall is well preserved at the healed stage.
Finally, the etiology remains unknown. In this regard, however, we have a very strong clue in the astoundingly close relationship between the use of tobacco and the occurrence of the disease. If the patient absolutely abandons smoking, the natural history of the disease is uneventful.

L'analyse de 115 malades atteints de maladie de Buerger observée au cours de ces 4 dernières années permet d'aboutir aux conclusions suivantes:

La maladie de Buerger est essentiellement une affection des petits vaisseaux et des vaisseaux de taille moyenne des extrémités. La maladie débute à la périphérie et s'étend en très peu de temps vers la poplitée d'une part et les artères de l'avant-bras d'autre part. Dans la majorité des cas, elle concerne 3 ou 4 membres. Bien que les premières manifestations de l'ischémie soient variables, elle débute le plus souvent par une lésion trophique.

Par rapport à la détérioration incessante de la circulation périphérique, l'extension vers la racine du membre est peu fréquente. La progression se fait de façon discontinue au niveau de l'artère principale.

Bien que la chirurgie artérielle radicale soit rarement pratiquée en raison des lésions distales, la revascularisation artérielle représente le moyen d'obtenir la cicatrisation d'une ulcération. Tous les efforts doivent être entrepris pour sauver le membre.

Des micro-abcès constitués par des cellules géantes, des cellules épithélioïdes et des leucocytes constituent la lésion initiale caractéristique. La paroi vasculaire est bien conservé au stade cicatriciel.

L'étiologie reste inconnue. Le tabac joue cependant un rôle important dans la génèse de la maladie car lorsque le malade cesse de fumer, l'évolution naturelle de l'affection est sans incident.
Resumen
Al analizar nuestras observaciones sobre 115 pacientes con enfermedad de Buerger en el curso de un período reciente de 4 años, se obtuvieron los siguientes resultados:

La enfermedad de Buerger es primordialmente una enfermedad de los vasos pequeńos y medianos de las extremidades. La enfermedad se inicia en la periferia y alcanza las arterias infrapopluíteas o del antebrazo dentro de un lapso breve de tiempo después del comienzo de los síntomas. La mayoría de los pacientes exhibe afección de 3 o 4 miembros. Aunque la primera manifestación isquémica es variable, la lésion trófica es el síntoma principal en el momento del primer examen.

En contraste con un deterioro incesante de la circulación periférica, la extensión proximal es menos frecuente. Las lesiones con interrupción segmentaria juegan un papel importante en la progresión proximal de la lesión obstructiva de la arteria principal.

No obstante que la cirugía arterial directa es infrecuentemente posible por razón del proceso oclusivo distal, la revascularización arterial exitosa es de la mayor efectividad en cuanto a la cicatrizatión de la ulceratión. Todos los esfuerzos deben concentrarse sobre el salvamento de la extremidad.

Los microabscesos, conformados por células gigantes, células epitelioides y leucocitos, dan una apaiencia muy característica a la lesión trombótica en la etapa temprana.

La etiología permanece desconocida. A este respecto, sin embargo, poseemos una clave muy poderosa en cuanto a la sorprendentemente cercana relación entre el uso del tabaco y la ocurrencia de la enfermedad. Si el paciente abandona en forma absoluta el hábito de fumar, su historia natural ocurre sin novedad.
