Tropical diseases of the small bowel

Abstract

Typhoid and tuberculosis still account for the bulk of small bowel diseases encountered in surgical practice in most parts of the tropical world. In the emergency setting, these conditions present with perforation or obstruction of the intestine. Against this background must be considered the occasional appearance of conditions that lead to perforation or obstruction in temperate countries, e.g., lymphoma and regional enteritis. Children with infestations with parasites such as ascaris present a puzzling picture with acute abdominal signs. Tropical diseases affecting the small bowel are thus predominantly preventable. Their continued prevalence among causes of an acute abdomen indicates an unrealistic allocation of priorities in the mass control of endemic disease.

Résumé

La typhoÏde et la tuberculose occupent encore une place considérable dans la pathologie de l'intestin grÊle qui se manifeste dans de nombreux pays tropicaux. Elles se traduisent souvent par une perforation ou une occlusion de l'intestin. D'autres affections des régions tempérées, comme le lymphome intestinal et l'entérite régionale peuvent se rencontrer et se manifester de la mÊme manière. Chez l'enfant les parasitoses intestinales comme l'ascaridiose peuvent donner lieu à des symptômes aigus abdominaux difficiles à interprÊter. Les maladies tropicales de l'intestin grÊle peuvent Être prévenues. Le fait qu'elles restent la cause fréquente d'accidents abdominaux aigus témoigne de l'insuffisance des modes de prévention des affections endémiques.

Resumen

La tifoidea y la tuberculosos todavía representan el mayor volumen de la patología del intestino delgado que se presenta en la práctica quirÚrgica en la mayoría de los países tropicales. En situaciones de emergencia se presentan como perforación u obstrucción del intestino. Frente a este panorama general es necesario considerar el caso ocasional que produce perforación u obstrucción en las zonas templadas del mundo, o sea el linfoma y la enteritis regional. En los niños la infestación por parásitos tales como los áscaris puede exhibir un cuadro enigmático con signos abdominales agudos. Las enfermedades tropicales que afectan al intestino delgado son, por consiguiente, fundamentalmente prevenibles. Su continuada prevalencia entre las causas principales de abdomen agudo indica una asignación no realista de prioridades en los programas de control masivo de las enfermedades endémicas.
