Motor and electrical activity of the gastroduodenal junction before and after submucosal pyloroplasty

Abstract

In 5 mongrel dogs, motor and electrical activity of the gastroduodenal wall was measured before and after submucosal pyloroplasty using 5 transducers and 5 bipolar serosal electrodes. The significant increase in the motility index (MI) by 105% (p<0.05), as well as the contractile activity by 231% (p<0.01), in the antrum preoperatively after feeding was not demonstrated postoperatively. In spite of this, gastric emptying was accelerated postoperatively by 15 minutes, to a total time of about 2 hours by radiological check with solid meal. Disturbances of motor and electrical patterns in the distal antrum and the duodenal cap that are seen after Finney and Heinike-Mikulicz pyloroplasty were not observed in the present study in which submucosal pyloroplasty was performed. Since the submucosal pyloroplasty protects the hypomuscular zone of the gastroduodenal junction, the reflex inhibition of the duodenal cap during large antrat contractions is not disturbed postoperatively.

Résumé

L'activité motorice et électrique de la paroi gastrique et duodénale a été étudiée avant et après pyloroplastie extra muqueuse chez cinq chiens bâtards.

L'augmentation significative de l'index de la motilité (MI) de 105% (p < 0.05) aussi bien que celle de l'activité contractile de 231% (p < 0.01) au niveau de l'antre avant l'intervention ne fut pas retrouvée en post opératoire. En dépit de ce fait le temps de l'évacuation gastrique fut réduit de 15 minutes ainsi que le montra l'étude du transit d'un repas d'aliments solides.

Les troubles de la motilité et de l'activité électrique au niveau de l'antre et de la partie mobile du ler duodénum qui sont constatés après l'opération de Finney ou l'opération de Heinecke-Mickulicz ne sont pas observés après une pyloroplastie extra muqueuse de bonne qualité. Cette dernière opération qui respecte la zone sous-musculaire de la jonction gastro-duodénale fait que les fortes contractions de la région antrale n'entraînent aucun trouble du fonctionnement du jeu duodéno-antral.
