Résumé
Après une étude exhaustive de la morphologie analytique des segments du cercle artériel du cerveau, une étude des calibres comparés de ces segments montre que:
-
- chez le foetus de 4 mois, après la fin du développement du cerveau et des artères cérébrales, tous les segments sont de calibres grêles et sensiblement égaux,
-
- après 4 mois, une certaine différenciation dans les calibres des segments apparaît au niveau des communications postérieures,
-
- la différenciation des calibres des segments n'a pas tendance à s'accuser dans les premières années de la vie,
-
- chez l'adulte, la plus grande anarchie apparente règne sur le plan des calibres comparatifs des segments du cercle.
Une étude morphologique de synthèse précise la notion du cercle dit ≪idéal≫ et de piliers ≪dépendants≫, du cercle dit ≪précaire≫ et de piliers ≪autonomes≫, de piliers ≪dominants≫ et de piliers ≪dominés≫ et montre que chacune des trois communications du cercle est une anastomose par convergence, morphologie normale possédant une physiologie normale.
L'étude fonctionnelle montre enfin que le cercle artériel est un appareil de rééquilibration des charges de voies artérielles d'apport au cerveau, charges susceptibles de variations lors des compressions de ces voies dans les mouvements de la tête. Elle montre aussi que l'anarchie apparente dans le calibre des segments du cercle de l'adulte relève d'un modelage progressif en corrélation précise avec les modalités des compressions des voies d'apport.
