Zusammenfassung
1306 Seren von 1087 unausgewählten Patienten eines gemischten internistischen Krankengutes wurden mit dem Fluoreszenz-Antiglobulintest und 431 Seren von 397 Patienten desselben Kollektivs mit dem Antiglobulin-Konsumptionstest auf das Vorliegen antinukleärer Faktoren untersucht. In 73 Seren von 49 Patienten wurden antinukleäre Faktoren mit dem Fluoreszenztest nachgewiesen. Die Ergebnisse waren mit großer Sicherheit reproduzierbar. Durch Verdünnungs-, Blockierungs- und Verlaufsuntersuchungen ließen sich “falsch positive” und “falsch negative” Befunde weitgehend ausschalten. Sämtliche antinukleäre Faktoren enthielten IgG-, rund 50% IgM- und 13% IgA-Globuline. Die höchsten durchschnittlichen Titer aller antinukleärer Faktoren sowie der IgG- und IgM-Faktoren wurden bei Patienten mit chronischen Lebererkrankungen, die der IgA-Faktoren bei Patienten mit einem Lupus erythematodes beobachtet. Die Ergebnisse des Konsumptionstests waren weniger gut reproduzierbar. Bezogen auf die Resultate des Fluoreszenztests betrug der Prozentsatz “falsch positiver” Befunde 26%, derjenige “falsch negativer” Befunde 7%. “Falsch positive” Ergebnisse traten besonders häufig bei Kranken mit einer Hypergammaglobulinämie auf. Eine Beziehung zwischen Titer der antinukleären Faktoren und Ausfall des Konsumptionstests ließ sich nicht zweifelsfrei herstellen.
Summary
1306 sera gained from 1087 patients chosen at random from a miscellaneous collective of internal patients were examined for the presence of antinuclear factors, employing the fluorescence-antiglobulin test, while 431 sera from 397 patients belonging to the same collective were screened employing the antiglobulin-consumption test. In 73 serum samples obtained from 49 patients, the antinuclear factors were assayed using the fluorescence test. The results obtained were reproducible with a high degree of accuracy. Dilution- and blocking-tests, as well as the observation of the course of the tests enabled “falsely positive” and “falsely negative” findings largely to be excluded. All of the antinuclear factors examined contained IgG-, and some 50% IgM-, and 19% IgA-globulins. The highest average titres of the total antinuclear factors, as well as those of the IgG- and IgM-factors were encountered in patients with chronic liver diseases, the highest average titre of IgA-factors in patients with lupus erythematosus. The results obtained with the consumption test were not so clearly reproducible. As related with the results gained in the fluorescence test, the percentage of “falsely positive” findings was 26%, and of “falsely negative” findings 7%. The occurrence of “falsely positive” results was particularly high in patients with hypergammaglobulinaemia. The existence of a relationship between the titres of the antinuclear factors and the outcome of the consumption test was not clearly established.
