A systematic study of the vascularisation of the pancreas

Essai de systématisation de la vascularisation sanguine du pancréas

Summary

The anatomy of the vessels of the pancreas is well known. However, some refinement is necessary in response to new requirements. In this study 40 specimens of pancreas plus duodenum were dissected after injecting the vessels with latex. Arteries and veins were measured and drawn. The origin and course of the vessels and their anastomoses were studied. If the vessels less than 0,5 mm in diameter are disregarded, it is seen that the vascularisation of the head of the pancreas can be systematised. Since the development of very selective arteriography and phlebography, two main types of artery and of vein have been discerned. Charts of the vessels for such an identification have been suggested. The two types of vessel are either terminal or anastomotic. Determination of the type of vessel can be useful: for the localisation of endocrine tumors of the pancreas by selective catheterisation; in making the decision for a limited operative intervention when immediate effective blood supply of the remaining tissue is important.

Résumé

L'anatomie des vaisseaux du pancréas est bien connue. Cependant, des précisions sont indispensables pour répondre à de nouveaux objectifs. Dans ce travail, quarante pièces duodéno-pancréatiques ont été disséquées après injections des vaisseaux par du latex. L'origine, la destinée des vaisseaux et leurs anastomoses, ont été relevées, leurs calibres ont été mesurés. Si l'on fait abstraction des rameaux de moins de 0,5 mm, on s'aperçoit que la vascularisation céphalique est systématisable. On peut décrire deux grands types artériels et deux grands types veineux qui peuvent être identifiés dès le début d'une artériographie ou d'une phlébographie hypersélectives. Des organigrammes pour une telle démarche sont proposés. Selon les types, les vaisseaux sont terminaux ou multianastomosés. La détermination des types vasculaires peut être utile pour la localisation de tumeurs endocrines du pancréas par cathétérisme hypersélectif ou dans le choix d'un geste opératoire limité pour lequel une suppléance immédiatement efficace est importante.
