Zusammenfassung
Bei 24 Patienten mit totalem AV-Block wurde die Unterschenkeldurchblutung venenverschlußplethysmographisch vor und nach Schrittmacherstimulation gemessen.
Bei einer mittleren Herzfrequenz von 38/min lag die Durchblutung in Ruhe und während reaktiver Hyperämie („peak flow“) statistisch signifikant tiefer als bei 100 Gefäßgesunden und 51 Arteriosklerotikern.
Nach Schrittmacherstimulation auf 72/min stiegen Ruhe- und Hyperämiedurchblutung statistisch signifikant auf fast normale Werte an.
Nach Steigerung der Schrittmacherfrequenz von 72/min auf 94/min setzte bei 5 von 11 Patienten eine weitere signifikante Erhöhung der Hyperämiedurchblutung ein.
Die Befunde zeigen, daß die Schrittmacherstimulation bei Patienten mit Coronarsklerose und totalem AV-Block zu einer besseren peripheren Durchblutung führt. Mit Hilfe der Venenverschlußplethysmographie ist es auf einfache Weise möglich, diejenige Herzfrequenz zu bestimmen, die für die Durchblutung in Ruhe und muskulärer Tätigkeit am geeignetsten ist. Die Untersuchungen sollen zum Problem der „optimalen Herzfrequenz“ einen Beitrag leisten.
Summary
Calf blood-flow was measured by venous occlusion plethysmography in 24 patients with complete heart block before and after pacemaker implantation.
At a mean heart rate of 38 beats/min blood-flow at rest and during reactive hyperemia (“peak flow”) was significantly reduced, comparing these results with 100 normal and 51 arteriosclerotic patients.
After pacemaker stimulation at a rate of 72 beats per minute blood-flow increased significantly, reaching almost normal values.
Increasing heart rate from 72 to 94 beats per minute reactive hyperemia blood flow was further elevated in 5 out of 11 patients.
These results show that in patients with complete heart block due to coronary arteriosclerosis pacemaker stimulation leads to a better peripheral circulation. Using venous occlusion plethysmography it is easily possible to find out the heart rate which is most suitable for both resting and exercise conditions (“optimal heart rate”).
