The guy's trial of treatments of “early” breast cancer

Abstract

Between 1961 and 1971, 370 patients with stages I and II breast cancer were assigned by random sample to receive either wide local excision or standard radical mastectomy. Patients in both groups received postoperative radiotherapy which, admittedly, would be considered less than optimal nowadays for patients with an intact axilla. Follow-up has been conducted for 5 years in all patients and for 10 years in about half of them. Wide local excision was followed by a higher incidence of local cancer recurrence. However, there was no significant difference between the 2 operations in 5-and 10-year survival. Furthermore, there was no significant difference between the 2 treatment groups in the incidence of distant metastases after 10 years, although patients with stage II cancer had a significantly higher incidence of distant metastases after 5 years (but not after 10 years).

Résumé

Entre 1961 et 1971, 370 patientes atteintes d'un cancer du sein aux stades I ou II furent traitées, soit par excision locale large, soit par mastectomie radicale. L'intervention chirurgicale a été déterminée par tirage au sort. Les malades des deux groupes reÇurent en outre une radiothérapie postopératoire qui, actuellement, serait certainement considérée comme insuffisante pour des patientes sans métastase ganglionnaire axillaire. Toutes les malades ont été suivies pendant 5 ans et la moitié d'entre elles pendant 10 ans. La fréquence des récidives locales est plus grande après excision locale large, mais la survie à 5 et à 10 ans ne diffère pas significativement pour les deux types d'interventions. A 5 ans (mais pas à 10 ans), la fréquence des métastases à distance est plus élevée pour les malades ayant un cancer au stade II. Mais, à 10 ans, la fréquence des métastases à distance n'est pas significativement différente dans les deux groupes thérapeutiques.
