Le biguanidi nel trattamento dell'obesita'. Nota 1. Ricerca clinica controllata con placebo, anoressanti, fenformina e metformina

Riassunto

E' stata condotta una ricerca controllata per valutare l'azione delle biguanidi (fenetil- e dimetilbiguanide) sulla riduzione del peso corporeo di soggetti obesi con normale o ridotta tolleranza agli idrati di carbonio. Dopo 130 giorni di trattamento la riduzione dell'eccesso ponderale è analoga a quella ottenuta conplacebo e anoressanti. Le biguanidi non esplicano alcun effetto sulla sensazione di fame; non sembrano infine esercitare influenze significative sul miglioramento della tolleranza agli idrati di carbonio, osservato dopo il calo ponderale.

Les AA. ont éffectué une recherche controlée pour la valutation de l'action des biguanides (fenetyl- et dimetyl-biguanide) sur la réduction du poids des sujets obèses avec une tolérance aux carbohydrates normale ou réduite. Après 130 jours de traitement la réduction de l'excés pondérale est analogue à celle obtenue avec unplacebo et des substances anoréxiques. Les biguanides n'ont aucun effet sur la sensation de faim; elles ne semblent pas excercer une influence déterminante sur l'amélioration de la tolérance aux carbohydrates, observée après la chute ponderale.

Resumen

Se realizó una investigación controlada para valorar la acción de las biguanidas (fenetil y dimetil-biguanida) sobre la disminución del peso corporal de sujetos obesos con una tolerancia normal o reducida a los hidratos de carbono. Después de 130 días de tratamiento, la reducción del exceso ponderal es análoga a la que se consigue conplacebo y con anorexantes. Las biguanidas no ejercen ningún efecto sobre la sensación de apetito; tampoco parecen ejercer influencias significativas sobre el mejoramiento de la tolerancia a los hidratos de carbono, observado después del descenso ponderal.

Zusammenfassung

Es wurde eine kontrollierte Untersuchung durchgeführt, um die Wirkung der Biguanide (Phenethylbiguanid, Dimethylbiguanid) auf die Reduktion des Körpergewichtes, von fettleibigen Patienten mit normaler oder reduzierter Kohlenhydrattoleranz einzuschätzen. Nach 130 Behandlungstagen entspricht die Reduktion des Gewichtsüberschusses derjenigen einesPlacebo und Appetitzüglers. Die Biguanide entfalten keine Wirkung auf das Hungergefühl, schließlich scheinen sie keinen signifikanten Einfluß auf die Verbesserung der Kohlenhydrattoleranz auszuüben, die man nach der Gewichtsreduktion beobachtet.

Summary

The effects of biguanides (phenethyl- and dimethylbiguanide) on the body weight reduction, were evaluated, by a controlled clinical trial, in obeses with normal or impaired glucose tolerance. Biguanides, after a 130 days treatment, caused a loss overweight as well asplacebo or anorectic drugs and of the same value; besides they had no appetite suppressant effect. At last, biguanides seem to exert no significant activity on the glucose tolerance improvement, observed in the obese patients after simple body weight reduction.
