Zusammenfassung
I. Die „chronische Hepatitis“ ist weder ätiologisch, noch symptomatisch ein einheitlicher Krankheitsbegriff.
II. Nach klinischen, biochemischen, immunologischen und histologischen Kriterien teilen wir die chronische Hepatitis in eine chronisch progressive, eine chronisch persistierende und eine chronisch reaktive Hepatitis ein.
III. Außer durch klinische, biochemische und histologische Kriterien zeichnet sich die chronisch progressive Hepatitis auch durch immunologische Befunde aus.
1. Die quantitative Bestimmung der Immunglobuline ergab für die chronisch progressive Hepatitis eine starke Vermehrung der IgG-Globuline, daneben eine geringe Vermehrung der IgA- und IgM-Globuline. Andere Formen der chronischen Hepatitis zeigen dazu unterschiedliche Immunglobulinverteilungsmuster.
2. Ferner findet man bei chronisch progressiver Hepatitis im Serum unspezifische immunologische Begleitphänomene und krankheitstypische immunologische Reaktionen in Form von zirkulierenden Antikörpern.
Diese humoralen Antikörper gehören zumeist dem Immunglobulin-G-Typus an. Selten findet man auch Antikörper vom IgM- und IgA-Typ.
Diese Antikörper zeigen durchweg keine strenge Species-und Organspezifität. Sie reagieren zudem mit allen subcellulären Fraktionen. Allerdings wurden auch organspezifische Antikörper gegen lösliche Cytoplasmaantigene aus Humanleber nachgewiesen.
3. Neben den humoralen Immunphänomenen gibt es bei der chronisch progressiven Hepatitis Hinweise für celluläre Immunreaktionen in Form dichter lymphocytärer Infiltrate in den Periportalfeldern.
IV. Die immunologischen Befunde bei chronischer Hepatitis stellen einen zusätzlichen Parameter zur Differenzierung verschiedener Formen der chronischen Hepatitis dar.
V. In der Pathogenese der chronischen Hepatitis müssen zwei Mechanismen diskutiert werden, ein chronischer Virusinfekt und ein Autoimmunpathomechanismus. Während der chronisch persistierenden Hepatitis wahrscheinlich ein Virusinfekt zugrunde liegt, ist bei der chronisch progressiven Hepatitis ein Autoimmunpathomechanismus naheliegend. Dabei kommen als pathogenetisch wirksame Agenzien humorale Antikörper, zirkulierende cytotoxische Antigen-Antikörperkomplexe und stimulierte Lymphocyten in Betracht. Eine Klärung dieser Zusammenhänge steht noch aus. Chronischer Virusinfekt und Autoimmunpathomechanismus schließen einander in der Pathogenese der Erkrankung jedoch nicht aus.
Summary
I. Concerning etiology and symptomatology, the chronic hepatitis is a heterogenous disease.
II. Using clinical, biochemical, immunological and histological parameters we divide the chronic hepatitis in a “chronic progressive hepatitis”, a “chronic persisting hepatitis”, and a “chronic reactive hepatitis”.
III. Besides the clinical, biochemical and morphological criteria the chronic progressive hepatitis is characterized by immunological findings.
1. The quantitative determination of the immunoglobulins showed high levels of IgG in chronic progressive hepatitis, while IgA and IgM were by far not as much elevated. Other forms of chronic hepatitis had different patterns of the immunoglobulins.
2. In the sera of patients with chronic progressive hepatitis we found unspecific immunological phenomenons as well as circulating antibodies, which were typical for the disease. The circulation antibodies against liver antigens in chronic progressive hepatitis belong in most cases to the IgG-class of immunoglobulins. In a few cases we also found antibodies of the IgM- and IgA-class.
These antibodies are not strongly specific for species and organs. Reactions could be found also with all of the subcellular fractions of human liver cells. Besides that, also organspecific antibodies, reacting with antigens from the soluble cytoplasmic fraction of human liver could be detected.
3. In chronic progressive hepatitis cellular immunological reactions must also be discussed because of the infiltration of the periportal parts of the liver with lymphocytes.
IV. The immunological findings in chronic hepatitis seem to be an additional parameter for the differentiation of different forms of chronic hepatitis.
V. In the pathogenesis of chronic hepatitis two mechanisms must be discussed, the chronic viral infection, and the mechanism of autoimmunity. The chronic persisting hepatitis seems to be a persisting viral hepatitis. For the chronic progressive hepatitis, on the other hand, an autoimmun mechanism can be possible. Pathogenetic relevant agents could be humoral antibodies, circulating cytotoxic antigen-antibody-complexes or stimulated lymphocytes.
Concerning the pathogenesis of chronic hepatitis, chronic viral infection and autoimmun disease do not exclude each other.
