Summary
Due to the increasing occurrence of Candida infections and the difficulties in diagnosing Candida mycoses on time, the serological aspects thereof have become of greater interest. The serology is both useful to confirm a clinical and after culture mycosis suspicion and to control mycosis risk in patients.
The fact that the cell wall antigens are exclusively glucoproteins and that the cytoplasmic antigens consist of proteins and glucoproteins, as well as the possibility to determine the normal titer in healthy subjects give way to better interpretation of the results. It is however most important to follow the dynamic evolution of the titer; a single measurement is for the most part of little value. Therefore, when Candidiasis is suspected, it is advisable to perform at least three assessments every 3–4 days and to simultaneously evaluate the antibodies against cell wall glucoprotein antigens and the antibodies against cytoplasmic protein antigens. In order to manage the mycosis risk in patients, the determination every week or every fortnight of the titer against the glucoprotein antigens by a sensitive method is adequate. When the titer raises, the assessment of antibodies against protein antigens has to be carried out.
A significant variation of the titers within the normal range titer may be caused by a mucous Candidiasis. A significant rise of the titer up to pathological values associated with the identification of antibodies against cytoplasmic protein antigens is considered to be a symptom ascertaining systemic Candidiasis. — False negative results are to be expected after immunosuppressive effects caused by drug therapy or of genetic origin.
The evidence of humoral antibodies is only of diagnostic value and has no signification with regard to Candida infection immunity.
Zusammenfassung
Durch die Zunahme von Candida-Infektionen und die Schwierigkeit, Candida-Mykosen rechtzeitig zu diagnostizieren, erlangte die Candida-Serologie große Bedeutung. Sie dient einerseits der Erhärtung der klinischen und kulturellen Verdachtsdiagnose, andererseits der Überwachung Mykose-gefährdeter Patienten.
Die Erkenntnis, daß in der Zellwand nur Glykoproteinantigene, im Zellinhalt Protein- und Glykoproteinantigene vorkommen, sowie die Bestimmung des Normaltiters bei Gesunden machen es heute möglich, die Befunde besser zu interpretieren. Von entscheidender Bedeutung ist jedoch die Titerdynamik. Eine einmalige Untersuchung ist meist wertlos. Es wird deshalb empfohlen, im Verdachtsfall mindestens 3 Untersuchungen im Abstand von 3–4 Tagen vorzunchmen, wobei gleichzeitig der Antikörper-Titer gegen die Glykoproteine der Zellwände sowie das Vorkommen von Antikörpern gegen die Eiweißantigene des Zellinhalts bestimmt werden sollten. Für eine Überwachung Mykose-gefährdeter Patienten eignet sich die Titerbestimmung mit einer gut empfindlichen Methode zur Erfassung der Antikörper gegen Glykoproteine in 1 bis 2wöchigem Intervall. Bei Ansteigen des Titers ist die Untersuchung auf Antikörper gegen Eiweißantigene angezeigt.
Signifikante Titerschwankungen haben auch dann eine Bedeutung, wenn sie sich innerhalb des „Normaltiters“ bewegen. Sie können durch eine Schleimhautmykose verursacht sein. Ein signifikanter Titeranstieg auf pathologische Werteund der Nachweis von Antikörpern gegen Eiweißantigene des Zellinhalts werden als sicheres Zeichen einer Systemmykose gewertet. — Falsch negative Befunde sind bei genetisch oder Therapie-bedingter Depression der Antikörperbildung zu erwarten.
Der Nachweis humoraler Antikörper ist nur diagnostisch verwertbar, über die Immunitätslage gegen Candida-Infektionen sagt er nichts aus.
