Zusammenfassung
1. In der vorliegenden Arbeit werden biochemische Befunde von 10 Kranken mit Pilzvergiftung (Amanita phalloides) mitgeteilt und mit Literaturangaben, insbesondere mit tierexperimentellen Untersuchungsergebnissen verglichen. Die Stoffwechselwirkungen des Toxins werden diskutiert.
2. Die Gesamteiweißkonzentration im Serum war durch die Hämokonzentration zu Beginn der Intoxikation erhöht. In den Elektrophoresediagrammen fand sich anfänglich eine Abnahme des Albumins und derγ-Globuline, deren Ursache eine eiweißverlierende toxische Enteropathie sein dürfte. Von der 2. Woche an erfolgte ein langsamer Wiederanstieg auf zum Teil stark erhöhteγ-Globulinwerte als Hinweis für einen Leberschaden.
3. Der Anstieg des Haptoglobins im Serum in den ersten Tagen ist nicht durch eine Hämokonzentration bedingt, sondern Hinweis auf das Vorliegen einer entzündlichen Reaktionskonstellation in der akuten Vergiftungsphase und spricht gegen eine generelle, schwere Hämolyse im Krankheitsverlauf der vorliegenden Pilzvergiftungen.
4. Die Leberparenchymschädigung wurde selten durch einen Bilirubinanstieg, aber fast regelmäßig durch eine Enzymaktivitätszunahme im Serum deutlich. Die Bilirubinkonzentration stieg nur bei 3 von 10 Kranken über 2 mg pro 100 ml Serum an, gesteigerte Transaminasenaktivitäten konnten bei 8 von 10 Patienten nachgewiesen werden. Weniger häufig waren Aktivitätssteigerungen der LDH und der DPF-Aldolase. Die Verlaufskontrolle des Serumenzym-Spiegels ergab eine „Zweiphasenkinetik“; ein Vergleich der durch Pilztoxine bewirkten Schädigung mit der Tetrachlorkohlenstoff-Vergiftung liegt nahe.
5. Cholesterin (gelegentlich auch die Phosphatide) im Serum fallen bei 5 Patienten in den ersten Tagen auf Werte-unter 120 mg-% ab. Der Abfall betrifft zunächst die Cholesterinesterfraktion (Estersturz!).
Die Cholesterinverminderung entspricht den bei dystrophischer Leberschädigung und bei dekompensierter Lebercirrhose beobachteten Werten. Sie überdauerte die Enzymaktivitätssteigerung und bildet ein spätes Zeichen für die abgelaufene Intoxikation.
6. Bei der quantitativen Aminosäuren-Analyse im Serum zeigte sich eine ausgeprägte Argininverminderung; in einigen Fällen war Arginin nur noch in Spuren im Serum nachweisbar. Bei einer letal verlaufenen Vergiftung mit Coma hepaticum war die Konzentration mehrerer Aminosäuren im Serum auf das 5–10fache der Norm erhöht.
7. Bei der Mehrzahl der Kranken bestand zu Beginn der Intoxikation eine metabolische, meist dekompensierte Acidose. Innerhalb von 3–4 Tagen normalisierte sich unter einer Therapie mit Lactat-Glucose-Infusionen und Nebennierenrindensteroiden der Säurebasenhaushalt.
Die Serumelektrolyte waren abgesehen von einer Hypokaliämie infolge der Durchfälle und einer nur gelegentlich auftretenden Hypochlorämie normal.
8. Die harnpflichtigen Stoffe (Kreatinin, Harnstoff) im Serum stiegen am 1.–3. Tag nach der Intoxikation meist an.
9. In den Erythrocyten der Kranken waren die ATP- und Methämoglobin-Konzentrationen sowie die Aktivität der Glucose-6-Phosphatdehydrogenase und in 8 der 10 Fälle auch die der Glutathionreduktase normal. Das intracelluläre Kalium war bei einigen Kranken vermindert. Bei allen Kranken fiel der Glutathion-Gehalt der Erythrocyten während der ersten Tage nach der Intoxikation und die Glutathion-Stabilität (Beutler-Test) ab. In den Erythrocyten traten vermehrt Heinz-Körper auf. — Bei zwei Kranken im Verwandtschaftsverhältnis von Cousinen lagen Sonderverhältnisse vor, hier wurde ein hereditärer Glutathionreduktasemangel nachgewiesen. Bei diesen beiden Patientinnen führte die Amanitin-Intoxikation auch ohne Acidose zum starken Abfall des Glutathions in den Erythrocyten und zur Heinz-Körperbildung.
Summary
1. The present work informs about the biochemical results of 10 patients with fungus-poisoning (amanita phalloides) and compare it with literary references, especially with experimental tests. The metabolic effects of the toxin are discussed.
2. The protein-concentration in serum increased by the haemoconcentration in the beginning of the intoxication. In the electrophoretic diagram a decrease of albumins and gamma globulins was found at first, as a result of a proteinlosing toxic enteropathy. From the second week a slow increase followed in part to much elevatedγ-Globulin values representative of a liver damage.
3. The increase of the serum haptoglobulin in the first days is not an effect of the haemoconcentration; it is a sign of a inflamed reaction constellation during the acute poisoning. There is no evidence for a general severe haemolysis during the present fungus-poisoning.
4. Characteristically for liver damage there was no elevated bilirubin concentration in serum, but an increased serum enzyme activity. Only in 3 of 10 patients the bilirubin concentration exceeded 2 mg per 100 ml serum. Increased transaminase activities were estimated in 8 of 10 patients and more frequent as an elevated LDH and DPF.-aldolase. The subsequent control of the serum enzym level demonstrated a „two phase kinetic“. A comparison of the alteration caused by fungus toxines with a CCl4-intoxication is obvious.
5. Cholesterol (sometimes also phospholipids) decreased below 120 mg per 100 ml serum in five patients during the first days. The serum cholesterol decrease firstly prefered the esterfied cholesterol fraction (“ester decrease”).
6. A strongly marked arginin reduction in serum appeared in the quantitative amino acid analysis. In certain cases arginin was demonstrable only in traces.
In one letal coursed poisoning with coma hepaticum the concentration of certain amino acids in the serum was increased five-to tenfold.
7. At the beginning of the intoxication the majority of the patients showed a metabolic, for the most part decompensated, acidosis in the blood. Within of 3 to 4 days the acid-base-metabolism became normalized under infusions of lactat-glucose and corticosteroids. The serum electrolytes were normal, apart from a hypokaliaemia in consequence of the diarrhoeas and a hypochloraemia in same cases.
8. The retention substances (creatinin, urea) in the serum increased during the first three days after the intoxication.
9. In the red blood cells the concentration of ATP and methaemoglobin was normal, also the activity of the glucose-6-phosphatdehydrogenase and of the glutathionreductase in 8 of the 10 cases. The intracellular potassium was decreased in certain patients. In all the patients the glutathion value of the red cells and the glutathion stability decreased and the Heinz-bodies increased during the first days after the intoxication. In two patients related as cousins existed special circumstances, here a hereditary lack of glutathionsreductase was proved.
In these two patients the amanitin intoxication led, although without acidosis, to a strong reduction of glutathion and to the formation of “Heinz-bodies” in the red blood cells.
