Zusammenfassung
Ohne auf therapeutische Maßnahmen einzugehen, wurden 5 verschiedene Lipoproteinmuster beschrieben, wobei angenommen wurde, daß jeweils ein verschiedenes, familiäres Krankheitsbild zugrunde liegt. Nach bisherigen Untersuchungen an einem großen Patientengut [16] hat sich dieses Einteilungsschema als nützlich hinsichtlich der zu ergreifenden Therapie sowie als notwendig zur Erforschung der zugrunde liegenden biochemischen Stoffwechselstörung erwiesen. Klinischen Untersuchungen, in denen über Erfolg oder Mißerfolg medikamentöser Maßnahmen bei Hyperlipämien berichtet wird, sollten Angaben, wie sie zur Erkennung dieser verschiedenen Hyperlipoproteinämien notwendig sind, nicht fehlen.
In diesem Zusammenhang unentbehrlich für Therapie und Prognose ist die Abtrennung des als Typ III bezeichneten Krankheitsbildes von der Gruppe sog. „Hypercholesterinämien mit begleitender Hypertriglyceridämie“. Die hier beschriebene Nomenklatur wird gegenwärtig in einer großen Zahl von Kliniken und Laboratorien angewandt und erlaubt die Übersetzung von komplizierten Stoffwechselerkrankungen in einfache Begriffe. Die Nützlichkeit eines solchen Systems steht außer Frage — nicht zuletzt im Hinblick auf die zukünftige Entdeckung möglicher Untergruppen.
Summary
Five patterns of hyperlipoproteinemia are described without giving details for treatment. This system is based on five major lipoprotein patterns which are defined by paper electrophoresis. For more specific diagnosis of hyperlipemia analysis of lipoproteins has been shown to be more useful than chemical determination of plasma lipid concentrations alone. Of great importance is the separation and definition of Type III hyperlipoproteinemia from disorders previously known as “familial hypercholesterolemia” or “essensential hyperlipemia”.
This system for phenotyping hyperlipoproteinemia as introduced byFredrickson in 1965 is now in use in a large number of laboratories and clinics. It allows the translation of complicated metabolic disorders into simple terms and is the necessary requirement for both further biochemical investigation of lipid transport mechanisms as for proper treatment of lipid transport disorders.
