Zusammenfassung
Bei 10 Patienten mit frischem Myokardinfarkt wurden hämodynamische Messungen und Bestimmungen der systolischen Kontraktionszeiten unter dem Einfluß einer akut eingeleiteten und — nach mittelschneller Aufsättigung — dauerhaft fortgeführten Digitalisierung vorgenommen. Die Untersuchung erfolgte am 1., 2. und 3. Tag nach Infarkteintritt, außerdem am Ende der stationären Behandlung nach der 5.–6. Woche. Sie betraf jeweils Patienten ohne wesentliche klinische und hämodynamische Infarktauswirkungen. Zur Abgrenzung der Glykosidwirkung von unspezifisch ablaufenden Vorgängen der Herz-Kreislaufregulation wurde ein nicht digitalisiertes Patientengut mit akutem Myokardinfarkt in analoger Weise untersucht.
Dabei wurden folgende Befunde erhoben:

Unter der Wirkung von Digoxin ließen sich, verglichen mit der unbehandelten Patientengruppe, signifikante Änderungen von HZV, Schlagvolumen und peripherem Widerstand nicht nachweisen.

Eine Verkürzung der Austreibungszeit im Verlauf der ersten 3 Tage sowie eine anschließende stetige Verlängerung bis zum Ende der 5./6. Woche trat unabhängig von Glykosideinflüssen auf.

Im nicht digitalisierten Patientengut fand sich eine kontinuierliche Verlängerung der Druckanstiegszeit über den gesamten Untersuchungszeitraum. Diese Verzögerung der isovolumetrischen Druckentwicklung, die unabhängig vom Verhalten des Herzzeit- und Schlagvolumens als Ausdruck einer beginnenden kontraktilen Insuffizienz des Myokards aufzufassen ist, ließ sich durch Digitalis verhindern.
Die Ursachen für den positiven Glykosideffekt auf das frisch infarzierte Myokard sind in einer Steigerung der primär eingeschränkten Kontraktionskraft sowie einer Verbesserung der gestörten Kontraktionssynergie zu sehen. Es werden Kriterien diskutiert, nach denen sich die spätere Entwicklung einer manifesten Herzinsuffizienz als klinisches Endstadium eines defekten Kontraktionsablaufes durch eine frühzeitige und dauerhafte Digitalisierung abwenden läßt.
Summary
The hemodynamic effects of rapid and lasting digitalization following acute myocardial infarction were studied in 10 patients, none of them showing evidence of significant poor left ventricular function. The duration of the phases of ventricular systole were determined as well as the cardiac output and arterial pressure measured. Examinations were made on the first, second, and third day as well as towards the end of the patients' stay in the hospital during the fifth to sixth week. A control group of 10 patients with acute myocardial infarction, but without Digoxin was examinded in the same way.
The following results were obtained:

Compared with the untreated patients, no significant effects of Digoxin on cardiac output, stroke volume, and peripheral resistance could be established.

A shortening of the left ventricular ejection time during the first three days as well as a subsequent prolongation on to the fifth to sixth week could be proved independent from Digitalis.

Without digitalization patients showed a continuous increase of the duration of isovolumic contraction throughout the entire time of the examination. This prolongation of the time of rise of ventricular pressure could be prevented by Digoxin. Furthermore the mean systolic ejection rate increased significantly following digitalization, whereas it remained unchanged in the control group. Even without changes in stroke volume the prevention of deterioration of the contractile state of the heart is assumed to be of benefit in patients with acute myocardial infarction.
