Zusammenfassung
Bei Gesunden, Patienten nach Herzoperationen mit dem EKK, sowie allgemein bei Oligurien infolge hypovolämischen Schocks, Hämolyse etc. wurde die Wirkung einer 20%igen Mannitol-Lösung auf die Nierenfunktion untersucht. Dabei fand sich im Normalzustand und bei postoperativer Antidiurese eine Zunahme des Harnvolumens und der ausgeschiedenen Gesamt-Osmolarität um das 5–6fache und der NaCl-Ausscheidung um das etwa 3fache gegenüber den Ausgangswerten. Während die Nierendurchblutung bei Mannitol-Diurese um etwa 30% zunimmt, steigt das Glomerulumfiltrat nur geringgradig an oder bleibt unverändert. Vor allem die Ursache der vermehrten Natrium-Ausscheidung und Durchblutungszunahme werden eingehend diskutiert. Bei rechtzeitiger Infusion erzeugt Mannitol beim sogenannten funktionellen Nierenversagen (Harnzeitvolumen weniger 0,3 ml/min,U/P osm ∼ 1) nach den bisherigen klinischen Erfahrungen in über 50% aller Fälle eine ausreichende Urinausscheidung. Der protektive und therapeutische Effekt der selbst bei kleinsten Filtratmengen durch Mannitol möglichen Diurese wird anhand klinischer Beispiele besprochen. Bei jeder Oligurie ungeklärter Genese sollte zunächst eine Testdosis von 20%igem Mannitol injiziert werden (100 ml in 10 min). Bei einer Anurie infolge organischer Nierenschädigung ist auch durch Infusion von 20%igem Mannitol keine Wiederkehr der Diurese zu erwarten.
Von chirurgischer Seite ergeben sich für die Infusion von Mannitol folgende Indikationen:

Senkung des intracraniellen Druckes bei neurochirurgischen Eingriffen.

Therapie des Hirnödems.

3.
Prophylaxe der Oligurie bei traumatischem oder hypovolämischem Schock, Zusatz zum Perfusionsvolumen bei Operationen mit dem EKK, nach sonstigen größeren operativen Eingriffen, Transfusionszwischenfällen, hämolytischen Krisen und Verbrennungen.

Differentialdiagnostische Abgrenzung des funktionellen vom organisch bedingten Nierenversagen.
Summary
The effect of a 20% mannitol solution on renal function was studied in healthy persons, patients having undergone a heart operation with extra-corporeal circulation, as well as in patients with oliguria due to hypovolaemic shock, haemolysis, etc. Under normal conditions and during post-operative anti-diuresis we found an increase of the urine volume and of the excreted total osmolarity of 5–6 times the initial values, and of the NaCl-excretion of about 3 times the initial values. While renal blood flow during mannitol diuresis increases by about 30%, the glomerular filtration does so only slightly, or even remains unchanged. Above all, the cause of increased Na-excretion and increase of blood flow are being thoroughly discussed. According to our clinical experience up to now, mannitol produces a sufficient urine excretion during the so-called functional renal failure (urine flow 0.3 ml/min,U/P osm 1) in over 50% of all cases, if the infusion is given on time. The protective and therapeutic effect of a diuresis rendered possible by mannitol, even at lowest filtration rates, is being discussed by means of clinical examples. In each oliguria of unknown aetiology a test dose of 20% mannitol should be injected (100 ml in 10 minutes). In an anuria due to organic renal damage a diuresis cannot be expected, not even with a 20% mannitol solution. The following are the indications for a mannitol infusion from the surgical point of view:

Lowering of the intracranial pressure during neurosurgical operations.

Treatment of cerebral oedema.

3.
Prevention of oliguria during traumatic and hypovolaemic shock, addition to priming volume during operations with extracorporeal circulation, after all extensive operations, transfusion complications, haemolytic crises and burns.

4.
Differential diagnosis between functional and organic renal insufficiency.
