Zusammenfassung
Die steroidinduzierte Nebennierenunterfunktion hat ungeachtet ihrer relativen Seltenheit im Vergleich mit anderen unerwünschten Wirkungen einer Corticosteroidbehandlung wegen häufiger diagnostischer Schwierigkeiten und wegen öfter beobachteter Todesfälle schon immer besonderes Interesse gefunden. Durch eine vor allem in den letzten Jahren entwickelte differenziertere Funktionsdiagnostik des Hypothalamus-Hypophysenvorderlappen-Nebennierenrinden-Systems (HT-HVL-NNR-System) ist es möglich geworden, genauere Einblicke in die Pathogenese der steroidinduzierten Nebennierenrindenunterfunktion zu gewinnen. Daraus ergeben sich wiederum Folgerungen für die Prophylaxe und Therapie dieser Störung.
In der vorliegenden Übersicht wird versucht, die zahlreichen Einzelbefunde, vor allem der neueren Literatur (1964–1968) zusammenzufassen und einer einheitlichen Deutung zugänglich zu machen.
Nach einem kurzen Überblick über die Physiologie und Pathophysiologie des HT-HVL-NNR-Systems, die Klinik der steroidinduzierten NNR-Unterfunktion und die Diagnostik des Systems werden folgende Fragen ausführlicher besprochen:

Wann und unter welchen Steroiddosen ist mit einer Hemmung des Systems zu rechnen und welchen Einfluß haben Dosishöhe und Dauer der Behandlung auf die Restitutionsphase, falls eine Hemmung erfolgte.

2.
Läßt sich durch eine geeignete Dosierung, evtl. durch ein verändertes Dosierungsschema bei gleicher Wirkung auf die Grundkrankheit eine Hemmung des Systems vermeiden.

3.
Welche diagnostischen und therapeutischen Maßnahmen sind bei steroidhehandelten Patienten zu treffen, die operiert werden müssen oder die einer anderen schweren Stress-Situation ausgesetzt sind.

4.
Wie soll am besten hinsichtlich der Grundkrankkeit und der Nebennierenrindenfunktion eine Steroidbehandlung beendet werden.
Aus den Befunden können folgende Schlußfolgerungen gezogen werden:

Mit Prednisondosen bis zu 3 mg täglich wird auch bei sehr langer Applikationsdauer keine NNR-Unterfunktion erzeugt. Die Wahrscheinlichkeit, daß eine Hemmung der NNR-Funktion auftritt, nimmt zu mit steigenden täglichen Dosen. Oberhalb von 15 mg täglich wird nahezu bei jedem Menschen eine schwerere und länger anhaltende Störung bewirkt.

Es besteht wenigstens unter kleineren Dosen eine unterschiedliche, individuelle Empfindlichkeit gegenüber der supprimierenden Wirkung einer Steroidbehandlung.

Die Häufigkeit pathologischer Testergebnisse der NNR-Funktion nach einer Steroidbehandlung, die je nach der Art des Tests und der Dosishöhe unmittelbar nach Therapieende verschieden hoch ist, geht innerhalb weniger Tage zurück. Diese Tendenz der spontanen Erholung ist sehr deutlich beim basalen morgendlichen Plasmacortisolspiegel und auffallend gering beim Metopiron- und Insulinhypoglykämie-Test, vor allem nach einer hochdosierten Behandlung. Diese Diskrepanz der Erholungsgeschwindigkeit der einzelnen Funktionsprüfungen wird erläutert.

Die Behandlungsdauer einer Steroidbehandlung gewinnt vor allem dann an Bedeutung, wenn höhere Dosen Corticosteroide (über 15 mg Prednison täglich) über eine längere Zeit als 4–5 Monate verordnet werden. Es ist dann in etwa 40% der Fälle mit einer NNR-Atrophie zu rechnen, deren Restitution unter Umtänden viele Wochen bis Monate benötigt.

Die Häufigkeit einer steroidinduzierten NNR-Unterfunktion läßt sich durch Variation des Therapieschemas deutlich herabsetzen. Am günstigsten ist eine alternierende Behandlung (Gesamtdosis jeden 2. Tag morgens). Länger wirkende Corticosteroide wie Dexamethason und Betamethason sind für eine solche Therapie nicht geeignet.

Hinsichtlich der Hemmung des HT-HVL-NNR-Systems können langwirkende orale und intramuskuläre Depot-Steroidpräparate nicht empfohlen werden.

Vor und während Stress-Situationen (z. B. Operationen) sollten zusätzlich Corticosteroide gegeben werden, wenn die Patienten noch unter der Behandlung stehen oder wenn diese kürzer als 6 Monate zurückliegt. Vor einer evtl. notwendigen Operation ist es wünschenswert, sich wenigstens grob über die NNR-Funktion zu informieren. Es wird dafür eine einfache Testkombination von hoher diagnostischer Aussagekraft vorgeschlagen.

Aus Sicherheitsgründen ist es in den meisten Fällen zweckmäßig, eine Corticosteroidbehandlung ausschleichend zu beenden. Bei bereits eingetretener NNR-Atrophie ist die zusätzliche Verordnung von kleinen Dosen ACTH zur Verkürzung der Restitutionsphase angezeigt.
Summary
Although relatively uncommon as compared with other side effects with corticosteroid treatment, the steroid-induced adrenal insufficiency has aroused particular interest because of diagnostic difficulties and subsequent death following such therapy.
The several different test of hypothalamo-pituitary-adrenal [HPA) function now available facilitate a better understanding of the pathogenesis of the steroid-induced adrenal insufficiency, allowing proper conclusions with regard to prophylaxis and therapy.
This review attempts to summarize the widespread observations, especially of recent literature (1964 to 1968), and to elucidate different questions involved.
After a short introduction of the physiology and pathophysiology of the HPA system and of clinical aspects and diagnostic problems with the steroid-induced adrenal insufficiency, the following questions are discussed in detail:

When and with which corticosteroid dosis is suppression of the HPA system to be expected and to what extent is recovery affected by dosis and duration of treatment in case of suppression?

2.
Is it possible to reduce suppression by changing the mode of treatment without altering the therapeutic effect?
-
3.
Which diagnostic and therapeutic measures are necessary in patients to be treated surgically or submitted to other kinds of stress?
-
4.
Which is the best way to terminate long-term corticosteroid therapy with regard to the illness and function of the HPA system?
The following conclusions can be drawn from the results:

1.
A prednisone dosis up to 3 mg daily does not lead to suppression of the HPA system irrespective of duration of treatment; however, the likelihood of a suppressed adrenal function increases with larger doses. Over 15 mg prednisone produces a suppression in almost every individual.

2.
An individual sensitivity obviously exists with the suppression action of corticosteroid therapy.

3.
The frequency of abnormal results found when studying the HPA system following long-term corticosteroid treatment, which vary in the different tests immediately after cessation of therapy, decreases during the following days. This tendency is most apparent with the basal morning plasma cortisol level and remarkably less obvious with the insulin hypoglycemia and metyrapone tests, especially following treatment with a higher dosis. This discrepancy in the speed of recovery of the different tests is discussed.

The duration of treatment requires attention when a larger dosis than 15 mg prednisone daily is administered over more than 4–5 months. Under such circumstances adrenal atrophy can be expected in about 40% of the patients treated. Recovery time may then take up to several months.

Steroid-induced adrenal insufficiency can greatly be diminished by changing the mode of treatment. Most effective is an alternate day therapy (total dosis every other morning). Longer acting drugs, such as betamethasone and dexamethasone, are not suitable for such therapy.

Pharmacological preparations of corticosteroids with depot effect cannot be recommended for long-term corticosteroid therapy.

Before and during stress situations, e. g. operations, a corticosteroid cover should be given if the patients are on treatment or this has been completed less than 6 months ago. It is desirable to get at least a rough idea of the patients HPA function prior to operation. For this purpose a simple screening test is proposed.

To be on the safe side it is advisable to complete long-term corticosteroid treatment by tapering off the dosis. If it becomes evident that during therapy adrenal atrophy has occurred, the simultaneous administration of small amounts of ACTH is recommended.
