Zusammenfassung
Es wird über eine 51jährige Patientin mit Collum-uteri-carcinom(II) berichtet, bei der unter einer kombinierten Behandlung mit Radium und Röntgenpendelkonvergenzbestrahlung eine schwere Hämolyse auftrat.
Als Ursache der strahleninduzierten Hämolyse wurde ein Mangel an reduziertem Glutathion (GSH) in den roten Blutzellen nachgewiesen. Alle Enzyme die an der Glutathionreduktion beteiligt sind, hatten in den Erythrocyten normale Aktivitäten, so daß es sich — auch auf Grund der Serumaminosäurenchromatogramme — wahrscheinlich um eine Glutathionsynthesestörung handelt.
In den Erythrocyten fanden sich neben der Verminderung des GSH-Gehaltes um etwa 50% der Normalwerte in wechselnder Ausprägung eine Glutathioninstabilität und verstärkte Innenkörperbildung im Acetylphenylhydrazininkubationstest nachBeutler.
Die Glykolyse und der ATP-Gehalt der Erythrocyten waren normal, der Coombs-Test und Nachweis von Kälteagglutininen verliefen negativ.
Familienuntersuchungen ergaben, daß der GSH-Mangel hereditär ist und in dieser Familie wahrscheinlich autosomal rezessiv vererbt wird.
Die strahleninduzierte Hämolyse besserte sich gut unter einer Behandlung mit Nebennierenrindensteroiden und Bluttransfusionen. 6 Monate nach der Hämolyse war das rote Blutbild ohne weitere Therapie kompensiert, während die Thrombocyten und Leukocyten noch deutlich vermindert waren.
Summary
A case report is given of an 51 year old woman, suffering from carcinoma of the collum uteri (II), who developed a severe hemolysis during a combined treatment of X-ray and radium radiation.
This hemolysis, caused by radiation, was produced by a low level of reduced glutathion (GSH) in the red cells.
All enzymes of the red cells, involved in the reducing reactions of glutathion, showed normal activities. Thus a defect in glutathion synthesis is most likely. The results of the amino acid chromatography were in accordance to this hypothesis.
In the erythrocytes the level of the GSH content amounted only to 50% compared to normal individuals; beside this an instability of glutathion to a verying degree was found and an increased “Heinz body formation” after incubation with acetyl-phenylhydrazine (Beutler). Glycolysis and ATP-content of the red cells were within normal limits, cold agglutinins and coombs-test were negative. The investigation of other members of the same family showed the same diminution of GSH in the red cells. Most likely the inborn metabolic disorder is inherited by an autosomal recessive way.
The hemolysis was treated succesfully by blood transfusions and adrenal steroids.
Six months after the onset of the hemolysis red cell count and hemoglobin were compensated without further therapy, where as leucocytes and blood platelets were still diminished.
