Zusammenfassung
1. Bei 17 Kranken mit der Symptomatik einer ausgeprägten cerebralen Arteriosklerose wurde die Hirndurchblutung mit der Stickoxydulmethode vonKety undSchmidt bestimmt. Gleichzeitig erfolgten Analysen des cerebralen Sauerstoff-und Glucoseverbrauchs, sowie der Abgabe von Kohlensäure, Milchsäure und Brenztraubensäure aus dem Gehirn. Enzymatische substratspezifische Meßmethoden kamen zur Anwendung.
2. Die Hirndurchblutung betrug 38,7 ml/100 g·min, statt normalerweise 55,1 ml/100 g·min.
3. Die Aufnahme von Sauerstoff war mit 2,71 ml/100 g·min (statt normalerweise 3,71 ml/100 g·min) und die von Glucose mit 3,27 mg/100 g min (statt normalerweise 5,30 mg/100 g·min) statistisch signifikant herabgesetzt.
4. Vom Hirngewebe wurden 0,24 mg Milchsäure/100 g·min und 0,025 mg Brenztraubensäure/100 g·min ins venöse Blut abegeben. Der glykolytische Anteil am Gesamtglucoseumsatz des Gehirns betrug 6,8% (statt normalerweise 7,6%).
5. Der Quotient aus Glucoseverbrauch/Sauerstoffverbrauch war mit 1,23 im Vergleich zum Normalwert von 1,47 statistisch signifikant vermindert.
6. Die Glucosebilanz bei den Kranken mit Hirnarteriosklerose ist nicht ausgeglichen: Trotz der Verminderung sowohl des Sauerstoff- als auch des Glucoseverbrauchs entnimmt das Gehirn dem arteriellen Blut mehr Sauerstoff, als für die Verbrennung der Glucose notwendig wäre. Daraus ist zu schließen, daß die Arteriosklerose der Hirngefäße zu einer Stoffwechselstörung des Hirngewebes führt, bei der neben Glucose noch andere Substrate, vermutlich Aminosäuren und Lipoproteide abgebaut werden, die normalerweise keine Bedeutung für den oxydativen Stoffwechsel der Hirnzellen haben.
Summary
1. The cerebral blood flow of 17 patients with cerebral arteriosclerosis was measured by the nitrous oxide method ofKety andSchmidt.
2. The concentration of glucose, lactate and pyruvate in arterial and cerebral venous blood was determined by enzymatic methods. For the determination of glucose the Hexokinase-Glucose-6-phosphatdehydrogenase reaction was used. Lactate and pyruvate were determined by use of Lactatdehydrogenase. Oxygen and carbon dioxide were analysed by the van Slyke manometric technique.
3. The cerebral blood flow was 38,7 ml/100 g·min (normal value 55,1 ml/100 g·min).
4. The cerebral oxygen consumption was 2,71 ml/100 g·min (normal value 3,71 ml/100 g·min).
5. The cerebral glucose uptake was 3,27 mg/100 g min (normal value 5,30 mg/100 g min).
6. The delivery of lactate to the venous blood was 0,24 mg/100 g·min (normal value 0,42 mg/100 g·min). The total amount of pyruvate given up by the brain was 0,025 mg/100 g·min (normal value 0,055 mg/100 g·min). 6,8% of the glucose was metabolized by glycolysis (normal value 7,6%).
7. The quotientQ of glucose consumption/oxygen consumption was 1,23 (normal value 1,47).
8. In cerebral arteriosclerosis the brain takes up more oxygen than would theoretically be required for the oxidation of glucose, although the oxygen consumption and the glucose consumption are both reduced. It is assumed that in cerebral arteriosclerosis the oxidative metabolism of the brain cells must be at least partially supported by some substances other than glucose.
