Zusammenfassung
Es wird über eine neue inulinartige Substanz, Polyfructosan-S (PFS), berichtet, die gegenüber Inulin (In) einige Vorteile hat (völlige Löslichkeit in kaltem Wasser, Alkalistabilität, geringere Hydrolysegeschwindigkeit). Da die chemische Analyse von PFS in Urin und Plasma mit derjenigen von In identisch ist, können vergleichende Untersuchungen über die renale Clearance und das Verteilungsvolumen von PFS mit In nicht simultan durchgeführt werden.
Bei 12 erwachsenen Personen mit und ohne Störungen der Nierenhämodynamik findet sich unter individuell standardisierten Basalbedingungen im Wasser-, Salz- und Eiweißhaushalt ein Clearanceverhältnis PFS/In von 1,016±0,099 (Spanne: 0,91 bis 1,22). Zwischen nierengesunden und nierenkranken Personen besteht kein Unterschied hinsichtlich der Abweichungen vom Mittelwert. Das bei denselben Probanden in Kombination mit der renalen Clearance bestimmte Verteilungsvolumen (VV) von PFS entspricht demjenigen von In(VVPFS/VVIn=1,005±0,093; Spanne: 0,84–1,19). Bei Erhöhung der Plasmakonzentration von PFS von 21,5 auf 104,8 (durchschnittlich von 28,0–72,1) mg/100 ml bleibt die PFS-Clearance bei insgesamt 12 nierenkranken und nierengesunden Personen individuell konstant. — Toxische oder pyrogene Reaktionen bzw. andere nachteilige Nebenwirkungen von PFS werden bei 48 Personen, auch bei hohen Plasmakonzentrationen, nicht beobachtet.
PFS eignet sich zur Bestimmung des Glomerulusfiltrates und des physiologisch aktiven extracellulären Flüssigkeitsvolumens beim Menschen ebensogut wie Inulin.
