Summary
The effect of Bisacodyl on water and electrolyte movement in the human colon was studied in 5 persons by intestinal perfusion the tube being placed into the coecum. The perfusate contained Na (140 meq/l) as chloride and bicarbonate, K (2.8 meq/l) as chloride and14C-PEG as a non-absorbable marker. It was perfused at a rate of 10 ml/min and collected via rectal tube.
From this solution an average of 1 ml water and 200 µeq/min Na were absorbed while 20 µeq K/min was entering the intestinal lumen. After 1 mg-% Bisacodyl was added to the perfusate Na and water absorption was reversed, an average of 2 ml water and of 300 µeq Na/min entering the lumen. The K-secretion increased up to 95 µeq/min. When the perfusion was continued with Bisacodyl-free solution, the effect of Bisacodyl was reversable although the original values were not reestablished 2 hours later. About 10% of the perfused Bisacodyl was excreted in the urin. Bisacodyl was found to be glucuronized in urin and bile which suggests its absorption from the colon and glucuronization in the liver.
Bisacodyl, a compound of the diphenolic laxative group thus induces a net movement of fluid into the colon and therefore belongs to the group of substances which induce secretion into the colon such as the bile acids and hydroxy fatty acids. The pronounced increase of K-secretion into the colon may explain the clinical observation of K impoverishment of the organism in chronic laxative abuse.
Zusammenfassung
Bei 5 Versuchspersonen wurde die Wirkung von Bisacodyl auf die Wasser- und Elektrolytbewegungen im menschlichen Colon untersucht. Das Colon wurde durch eine Sonde perfundiert, die in das Coecum plaziert wurde. Die Perfusionslösung enthielt NaCl (140 mäq/l) als Chlorid und Bicarbonat und K (2,8 mäq/l) als Chlorid sowie14C-Polyäthylenglykol als Marker. Sie wurde mit einer Rate von 10 ml/min perfundiert und über ein Darmrohr im Rectum wieder aufgefangen.
Aus dieser Standardlösung wurden durchschnittlich 1 ml Wasser und 200 µäq Na resorbiert, während 20 µäq K in das Darmlumen einströmten. Nach Zusatz von 1 mg-% Bisacodyl zum Perfusat kehrte sich die Resorption von Wasser und Na um: der Nettoeinstrom betrug durchschnittlich 2 ml Wasser und 300 µäq Na/min. Die K-Sekretion steigerte sich bis auf durchschnittlich 95 µäq/min. Bei Fortführung der Perfusion mit bisacodylfreier Lösung war dieser Effekt des Bisacodyls reversibel, ohne daß jedoch nach 2 Std die Ausgangswerte ganz wieder erreicht wurden. Etwa 10% des perfundierten Bisacodyl wurden im Urin wieder ausgeschieden. Bisacodyl lag sowohl im Urin als auch in der Galle fast ausschließlich in glucuronierter Form vor, was auf seine Resorption und Glucuronierung in der Leber schließen läßt.
Bisacodyl, eine Verbindung aus der Gruppe der diphenolischen Laxantien, führt somit zu einem Flüssigkeitseinstrom in das Colon und ist damit unter die Gruppe der sekretionsfördernden Substanzen wie Gallensäuren und Hydroxyfettsäuren einzuordnen. Die besonders ausgeprägte Verstärkung des K-Einstroms in das Colon könnte die klinische Beobachtung der K-Verarmung des Organismus bei chronischen Laxantienabusus erklären.
