Summary
The effect of a therapeutic 500 mg tetracycline dose on the59Fe-absorption from a therapeutic 50 mg59Fe2+ dose was studied in subjects with normal and depleted iron stores by intraindividual comparison. The whole body retention of absorbed59Fe was measured within the 4 π counting geometry of a large volume radioactivity with a liquid organic scintillator. If administered together in aqueous solution tetracycline produced an inhibition of59Fe2+-absorption of 78±19% (\(\bar X\) a±S.D.) in subjects with normal iron stores and an inhibition of 65±14% in subjects with depleted iron stores (pre-latent iron deficiency). The simultaneous administration in separate stick capsules did also result in a significant although less pronounced inhibition of the iron absorption (32±20 and 40±8.4%, respectively). Tetracycline is another potent inhibitor of iron absorption and the formation of stable iron-tetracyline-chelates explains the interference between therapeutic doses of iron and tetracycline, During tetracycline therapy iron or other metallic cations-containing preparations should never be administered concomitantly. A time interval of about 3 hours however is sufficient to avoid drug interaction between iron and tetracyclines.
Zusammenfassung
Die Beeinflussung der59Fe-Absorption aus therapeutischen 50 mg59Fe2+-Mengen durch in der Therapie übliche 500 mg Tetracyclin-Mengen wurde an Personen mit normalen und erschöpften Eisenreserven untersucht und die Gesamtkörperretention des absorbierten59Fe in der 4 π-Geometrie eines Großraum-Radioaktivitätsdetektors mit flüssigem organischem Szintillator gemessen. Bei gleichzeitiger Verabfolgung in wäßriger Lösung hemmten 500 mg Tetracyclin die59Fe-Absorption aus 50 mg59Fe2+ um 78 ± 19% (\(\bar X\) a±S xi) bei Personen mit normalen Eisenreserven und um 65±14% bei Personen mit erschöpften Eisenreserven (prälatenter Eisenmangel). Nach gleichzeitiger Verabfolgung in getrennten Steckkapseln wurden mit 32±20% bei normalen Eisenreserven und 40±8% bei erschöpten Eisenreserven etwas geringere Hemmeffekte des Tetracyclin auf die Eisenabsorption festgestellt. Wahrscheinlich ist die Tetracyclin-Hemmung der Eisenabsorption auf die auch in vitro nachweisbare Bildung stabiler, schlecht absorbierbarer Eisen-Tetracyclin-Komplexe zurückzuführen. Die gleichzeitige orale Verabfolgung von Tetracyclin- und Eisen oder andere Metallkationen enthaltenden Präparaten ist zu vermeiden. Ein etwa dreistündiger Abstand zwischen oraler Eisen- und Tetracyclin-Applikation genügt jedoch, um die aus der Bildung der Eisen-Tetracyclin-Chelate resultierende gegenseitige Hemmung der Tetracyclin- bzw. Eisen-Absorption zu vermeiden.
