Zusammenfassung
An 33 Hunden und 35 Patienten wurden experimentelle und klinische Studien in tiefer Anaesthesie und tiefer Hypothermie mit Operationen am offenen Herzen angestellt. Die Autoren haben ihre eigene Anaesthesiemethode zur Herbeiführung der tiefen Hypothermie mitgeteilt. Sie ist die sicherste Methode für Hypothermie, die sie kennen. Bei 35 klinischen Fällen (13 Vorhofseptumdefekte, 11 Ventrikelseptumdefekte, 1 Pulmonalstenose, 2 Fallotsche Trilogie, 2 Fallotsche Pentalogien und 6 Mitralstenosen) wurden sicher und erfolgreich unter einem Herz-Kreislaufstillstand von 6 min 30 sec bis zu 42 min 35 sec zwischen 25°C und 18°C Operationen am offenen Herzen durchgeführt. Die Dauer der Herzmassage lag zwischen 10 sec und 30 min 49 sec. Die Ergebnisse beweisen, daß die bisherige Annahme, daß die Sicherheitsgrenze bei einer Temperatur von 30°C und einem Herzkreislaufstop von etwa 8 min liege, revidiert werden kann. Die Ergebnisse erlauben weiterhin die Annahme, daß eine Hypothermie von unter 20°C auch am Menschen angewandt werden kann, was erlauben würde, den Kreislaufstillstand auf 1 Std und darüber auszudehnen, falls eine Mischung von Kalium und Magnesium zur Herbeiführung eines kompletten Herzstillstands verwendet wird. Damit könnten die meisten Herzerkrankungen sicher in tiefer Hypothermie operiert werden.
