Visual damage after removal of hypophyseal adenomas: Possible importance of vascular disturbances of the optic nerves and chiasma

Summary

Review of the literature on the possible causes of visual loss after removal of hypophyseal adenomas. Report of two personal oases of this sequela, one of early and the other of late onset. The follow-up of both cases and the findings at re-operation in one of them seem to support the view that visual damage may depend upon impairment of blood supply to the optic nerves and chiasma caused by the surgical manoeuvres in the neighbourhood of them.

Zusammenfassung

Die Autoren geben zunächst eine Literaturübersicht über die möglichen Ursachen von Sehstörungen nach der Entfernung von Hypophysenadenomen. Anschließend berichten sie über zwei eigene Fälle, bei denen einmal die Sehstörung frühzeitig und im anderen Fall erst später einsetzte. Der weitere Verlauf beider Fälle und die Befunde bei einer Nachoperation des einen Falles scheinen die Ansicht zu stützen, daß die Sehstörungen auf einer Beeinträchtigung der Blutversorgung der Sehnerven und des Chiasmas beruhen, die durch die chirurgischen Maßnahmen in der Nachbarschaft dieser Strukturen ausgelöst werden.

Revue de la littérature sur le sujet. Exposé de 2 cas personnels avec cette complication, l'une d'apparition immédiate, l'autre d'apparition tardive. Les suites et les constatations à la réintervention de l'un d'entre eux permettent de penser que l'idée selon laquelle les dommages visuels dépendent de l'atteinte de la vascularisation du nerf optique et du chiasma provoquée par les manoeuvres chirurgicales réalisées dans leur voisinage doit étre retenue.

Riassunto

Rassegna délia letteratura sulle possibili cause di danno visivo dopo asportazione di adenomi ipofisari. Resoconto di due casi personal! di questa sequela, ad inizio precoce nell'uno e tardivo nell'altro. Le osservazioni postoperatorie in entrambi i casi e le constatazioni al reintervento in uno di essi sembrano avvalorare l'ipotesi che il danno visivo possa dipendere da compromissione dell'irrorazione dei nervi ottici e del chiasma causato dalle manovre chirurgiche in loro prossimità.

Resumen

Se hace una revisión de la literatura de las posibles causas de pérdida visual después de la extirpación de adenomas de la hipófisis. Se revisan dos casos personales de esta secuela, uno de aparicion precoz y el otro de aparición tardía. La evomción de ambos casos y los hallazgos en la reoperación de uno de ellos nos hacen suponer que la pérdida de la vision puede depender de la elevación del aporte sanguíneo a los nervios ópticos y al quiasma, ocasionado por las maniobras quirÚrgicas en los alrededores de ellos.
